Analysis on the correlation between killer immunoglobulin-like receptor gene polymorphism and cervical cancer

摘要 目的探讨杀伤细胞免疫球蛋白(Ig)样受体(KIR)基因多态性与宫颈癌的相关性。方法收集167例宫颈上皮内瘤变3级(CIN3)/宫颈癌患者和163例年龄匹配的无血缘关系健康女性外周血标本。采用SYBR GreenⅠ实时荧光PCR分析KIR基因及基因型频率在两组人群间的差异。结果 CIN3/宫颈癌组的KIR2DL2和2DS2基因频率低于健康人对照组(10.2%vs28.8%,P=0.000;9.6%vs 25.2%,P=0.000)。KIR2DL2/2DS2共表达个体的频率在病例组低于健康人对照组(9.0%vs22.7%,P=0.001),与降低疾病的患病风险有关(OR=0.336)。基因型AA1和BX8与增加CIN3/宫颈癌的患病风险有关(49.1%vs 35.6%,OR=1.746,P=0.013;10.8%vs 4.3%,OR=2.692,P=0.026),而BX4与降低疾病的患病风险有关(2.4%vs 7.4%,OR=0.309,P=0.036)。结论 KIR2DL2/2DS2对宫颈癌的发生可能具有保护性作用。 Objective To explore the correlation between killer immunoglobulin-like receptor (KIR) gene polymorphism and cervical cancer. Methods Peripheral blood samples were collected from 167 patients with cervical intraepithelial neoplasia 3 ( CIN3 ) or cervi-cal cancer and 163 age-matched healthy women. Then, K/R gene and genotypic frequency in the two groups were analyzed by SYBR Green I real-time PCR genotyping assay, and the results were compared. Results The gene frequencies of K/R2DL2 and 2DS2 were significantly lower in the CIN3/cervical cancer group than in the control ( 10.2% vs 28.8%, P = 0. 000; 9.6% vs 25.2%, P = 0. 000). The frequency of KIR2DL2/2DS2 coexpression in the CIN3/cervical cancer group was also significantly lower than that in the control (9.0% vs 22.7% , P =0. 001 ). Moreover, higher KIR2DL2/2DS2 frequency was associated with the lower risk of CIN3/cervi-eal cancer ( OR = 0. 336 ). Inversely, the genotype AA1 and BX8 were associated with the increased risk of CIN3/cervical cancer (49.1% vs 35.6% , OR = 1. 746, P =0. 013 ; 10.8% vs 4.3% , OR =2. 692, P =0.026). In addition, the genotype BX4 was asso-ciated with the decreased risk of CIN3/eervical cancer (2.4% vs 7.4% , OR = 0. 309, P = 0. 036 ). Conclusion The genotype KIR2DL2/2DS2 may play a role in preventing the genesis and development of cervical cancer.
