改良QB-I灌注法在输尿管上段结石输尿管镜钬激光碎石术治疗中的应用

Modified QB- I perfusion applied in ureteroscope holmium laser lithotripsy in treatment of upper ureteral calculi

目的探讨改良QB-I灌注法在输尿管上段结石(UUC)输尿管镜钬激光碎石术(URHL)中的应用价值。方法将98例实施URHL治疗的UUC患者随机分为两组,A组48例采用高挂法灌注碎石,B组50例采用改良QB-I法灌注碎石,观察两组碎石率及术后并发症发生情况。结果 B组碎石成功率、结石排净率均高于A组(P<0.05);B组结石上移率、并发症发生率低于A组(P<0.05);两组手术时间比较差异无统计学意义(P>0.05)。结论改良QB-I灌注法可确保冲洗液的流速及压力,促进结石排净,提高治疗成功率,值得临床推广适用。

Objective To investigate the value of modified QB - I perfusion applied in URHL in the treatment of UUC. Methods 98 ca ses with UUC treated by URHL were randomly divided into 2 groups, 48 cases in group A used the hanging method perfusion, while 50 cases in group B used modified QB - I method perfusion. We observed gravel rate and incidence of postoperative complications of 2 groups. Results Success rate and clearance rate of group B was higher than that of group A（ P 〈 0105 ） ; stones mobility rate and com plication rate of group B was lower than that of group A（ P 〈 0.05 ） ;operation time between 2 groups was not statistically significant （P 〉 0.05 ）. Conclusion Modified QB - I perfusion can ensure flushing fluid flow rate and pressure, promote stone clearance,improve the success rate of treatment, which is worthy of clinical promotion.
