钆特酸葡胺注射液致过敏性休克

Allergic shock caused by gadoteric acid meglumine injection

1例63岁男性患者，既往有青霉素过敏史。为进行增强磁共振成像检查，给予静脉注射钆特酸葡胺注射液20mL，1min后患者出现严重的药物过敏反应。患者主诉皮肤症状、呼吸困难、大汗，血压不能检测，出现短暂意识丧失。给予心电监护，测BP67～72／44～48mmHg，HR120～124次·min^-1，血氧饱和度为90％～94％，经会诊考虑为药品引起的过敏性休克。先后给予地塞米松注射液、多巴胺注射液、氨茶碱注射液、肾上腺素注射液、6％羟乙基淀粉（130／0．4）氯化钠注射液等药物治疗。约1h后患者血压上升至正常水平，其他各项生命体征平稳。

One 63-year-old male patient had the allergic history to penicillin. The patient received gadoteric acid meglumine injection 20 mL intravenously for enhanced magnetic resonance image （MRI） examination in our hospital. After one minute, the patient appeared the serious allergic reaction. The patient complained of skin allergic reaction with hard breath and heavy sweat. The blood pressure could not be detected and the patient lost his consciousness. The patient was sent to the emergency room immediately for first aid. The blood pressure was 67 - 72/44 - 48 mm Hg, heart rate was 120 - 124 beats per minute and SaO2 was 90% - 94%. The medical diagnosis was an anaphylactic shock. The patient was given the medical therapy with dexone injection, dopamine injection, aminophylline injection, epinephrine injection and 6% hydroxyethyl starch （130/0.4） sodium chloride injection, about 1 hour later, the patient＇s blood pressure raised to normal level and the life signs were stable.
