脑原发性淋巴瘤的MRI影像学特征

MR Imaging Features of Primary Intracerebral Lymphoma

目的:研究脑原发性淋巴瘤的MRI影像学表现,提高对该病的正确诊断率。方法:回顾性分析20例经病理证实的原发性脑淋巴瘤患者的MRI影像学表现,所有病例均行MRI平扫及增强扫描。结果:20例患者中,单个病灶14例,多个病灶6例;幕上病灶17例幕下病灶3例;共28个病灶,病理类型均为弥漫大B细胞型非霍奇金淋巴瘤。MRI平扫表现为T1WI为稍低信号,T2WI为稍高信号,少数为等信号;8例病灶内可见坏死囊性变,瘤周水肿相对较重,占位效应较重,中线结构移位较明显。MRI增强扫描,病灶大多呈明显均质强化。结论:原发性脑淋巴瘤MRI表现有一定的特征性,认真分析其影像学特点,结合临床可以给出倾向性诊断。

Objective： To analyze MRI features of Primary intracerebral Lymphoma and improve the accurate diagnostic rate of the disease. Method： MRI findings of 20 patients with pathologically proved Primary Brain Lymphoma were retrospectively analyzed. Result： There were 14 cases with single lesion and 6 cases with multiple lesions. The lesions of 17 cases distributed in supratentorium and of 3 cases in inferiortentorium. The total cases showed 28 lesions altogether. All cases were pathologically non-Hodgkin＇s B cell type. On MRI, slightly hypointense on Tl-weighted images and isointense or slightly hyperintense on T2-weighted images. There were severe peritumoral edema and space occupying effect of 8 lesions. Most lesions showed Obvious homogenous enhancement on MRI. Conclusion： MR! performance of primary brain lymphoma has certain characteristic, to analyze its imaging characteristics and combine with clinical orientation can form A diagnosis.
