26例良性前列腺增生并发膀胱结石的微创治疗 被引量:2

摘要 目的:探讨经尿道前列腺汽化电切术(TURP)联合超声气压弹道碎石治疗前列腺增生(BPH)合并膀胱结石的临床经验。方法:选取2008年9月-2010年10月本院治疗的BPH合并膀胱结石患者26例,均行TURP联合超声碎石治疗,观察手术治疗效果。结果:手术均一次性成功,术后无明显并发症。术后随访1~6个月,患者IPSS评分(8.0±2.2)分、生活质量评分(1.5±0.6)分、平均最大尿流率(Qmax)(14.74-113)mL/s。上述指标与手术前比较差异具有统计学意义(P〈0.05)。结论:TURP联合超声气压弹道碎石治疗BPH合并膀胱结石可以缩短手术时间、减少出血,有效改善患者排尿等临床症状及生活质量,是安全的微创手术方法,对高龄高危患者尤其适合。 Objective: To study the clinical safety and efficacy of transurethral resection of the prostate ( TURP ) combined ultrasonic pneumatic lithotripsy for the treatment of prostatic hyperplasia ( BPH )'with bladder calculi. Method: 26 patients of BPH with bladder calculi in September 2008- October 2010 in our hospital were selected , TURP combined ultrasonic iithotripsy in treatment. Observed the surgical treatment effect. Result: The surgery of all patients were successed, no significant postoperative complications occurred. In the 1-6 months follow-up, the IPSS score was (8.0 ±2.2 ) points, the quality of life score ( 1.5±0.6 ) points, the average maximum urinary flow rate ( Qmax ) ( 14.7± 1.3 ) mL/s. The above indicators compared with before the operation differences statistically significant ( P〈0.05 ) . Conclusion: The management including TURP combined ultrasonic pneumatic lithotripsy in the treatment of BPH patients with bladder stones can save the operation time, less blood loss voiume, shorter the time of hospital stay, and effectively improves patient urination and quality of iife. It is a safe, effective and minimally invasive surgical method, especially for high-risk patients.
