血清CEA,TGF-β1,VEGF和MMP-2联合检测在结直肠肿瘤中的意义 被引量:4

摘要 目的 探讨血清癌胚抗原(CEA)、转化生长因子-β1(TGF-β1)、血管内皮生长因子(VEGF)和基质金属蛋白酶2(MMP-2)联合检测在结直肠肿瘤中的意义,为结直肠肿瘤的诊断提供一种简单、快捷、准确、创伤小的实验方法.方法 选取经病理证实未进行手术或放化疗的结直肠癌患者165名,同时选取非肿瘤血清CEA升高病人125名,正常对照组100名,用酶联免疫吸附试验法检测患者血清CEA,TGF-β1,VEGF和MMP -2含量,对比分析三者数值是否有统计学意义.结果 结直肠癌患者血清CEA,TGF-β1,VEGF和MMP -2水平明显高于正常对照组和非肿瘤血清CEA升高组,差异有统计学意义(t值分别为53.78,35.92,40.93,30.09,32.89,27.34,19.55及9.88,均P〈0.01).血清TGF-β1,VEGF和MMP-2水平与血清CEA含量呈正相关(0.48,0.36和0.45,均P〈0.01).血清CEA,TGF-β1,VEGF和MMP-2联合检测可降低非肿瘤血清CEA升高组异常率,提高特异度.结论 血清CEA,TGF-β1,VEGF和MMP-2联和检测可以提高结直肠肿瘤的诊断率,为临床治疗提供依据,有望成为新的鉴别方法. Objective To explore the significance of combined detection of carcinoembryonic antigen (CEA), transforming growth factor-beta1 (TGF-β1), vascular endothelia growth factor (VEGF) and matrix metalloproteinase-2 (MMP-2) in colorectal cancer to provide the easy,quick,accuracy and micro-trauma experimental methods for the diagnostic of colorectal cancer. Methods Chose 165 patients with colorectal cancer in hospital and outpatient service from November 2009 to No-vember 2011. They had no operation or chemoradiotherapy by proving through pathology. At the same time, chose 125 pa-tients who had high plasma CEA and no cancer, and 100 normal control subjects from Physical Examination Center. Then colleced blood for 5ml of all the subjects with an empty belly in the morning. Detected the level of CEA, TGF-β1, VEGF and MMP-2 by enzymelinked immunosorbent assay, and contrasted three results for statistical significance to estimate the effec-tiveness. Results CEA, TGF-β1, VEGF and MMP-2 were elevating obviouly in patients with colorectal cancer compared with non-cancer patients with high plasma CEA and normal control subjects (P〈0.01). There were significantly difference-in statistics, and positive corelationship with the level of TGF-β1,VEGF, MMP-2 and CEA (0. 48,0.36,0.45 ;P〈0. 01). It could decrease the false positive rate of non-cancer patients with high plasma CEA,enhance specificity through combined de-tection of CEA,TGF-β1, VEGF and MMP-2. Conclusion Combined detection of CEA, TGF-β1, VEGF and MMP-2 could enhance diagnostic rate of patients with colorectal cancer, and provide evidence for clinical treatment. It could become new method for distinguishing.
