摘要
目的 分析广西三级医院临床化学检验项目变异系数(Coefficient of Variation,CV),了解广西三级医院临床化学室内质控现状,探讨合理设定临床化学质量规范.方法 分析广西50所三级医院2012年连续6个月临床化学17个项目在控数据的CV值,分析各项目不同质量规范条件下实验室分布情况并对不同质量规范进行比较.结果 ①大部分项目基于生物变异适当的质量规范和最佳质量规范允许CV值分别明显小于1/3 TEa和1/4TEa质量规范允许CV值;②大部分临床化学互认项目的 CV值达到了〈1/3TEa的要求,但BUN项目两水平CV值均〈1/3TEa的实验室比例分别只有59.57%和72.5%;Ca水平1CV值〈1/3TEa的实验室比例只有79.17%;③大部分项目基于生物变异适当的质量规范和最佳质量规范的实验室所占比例均明显低于1/3 TEa和1/4TEa质量规范的实验室所占比例.结论 广西壮族自治区三级医院临床化学室内质量控制在控数据CV值总体上是满意的,各实验室应选择合适的质量规范进行使用,并通过实验室质量规范的持续改进不断提高检验质量水平.
Objective To analyse the Guangxi tertiary hospital clinical chemistry test items coefficient of variation (Coefficient of Variation,CV),know the current situation of clinical chemistry internal quality control and explore the setting of quailty specifications for clinical chemistry. Methods Analysed 50 tertiary hospital laboratories in Guangxl for six consecutive months in 2012 clinical chemistry 17 items in the CV value of the control data, analysed the various items of different quality specificationsunder the conditions of the laboratory distribution and compared different quality specifications. Results The allowing CV values based on the biological variability in the appropriate quality specifications and best quality specification were significantly less than 1/3 TEa, and 1/4TEa quality specification allows CV values in majority of the items. The CV value of majority clinical chemistry items reached〈1/3TEa quality specification requirements, But two levels of BUN CV values 〈1/3TEa laboratory proportion were only 59.57% and 72.5 %,Ca level1 CV value〈1/3TEa laboratory proportion was only 79.17%. Majority of the items the proportion of the appropriate and best quality specification laboratory based on biological variation were significantly less than the proportion of 1/3 TEa and 1/4TEa quality specification. Conclusion The clinical chemistry IQC CV values were satisfied in tertiary hospital laboratories of Guangxi,each laboratory should select the appropriate quality specifications for use,continuously improve the quality of test levels through continuous improvement of laboratory quality specifications.
