经尿道前列腺电切治疗高龄良性前列腺增生症40例疗效观察

Transurethral Resection of Prostate to Treat Eldly Patients with BelIign Prostatic Hyperplasia

目的：评价经尿道前列腺电切（TURP）治疗高龄良性前列腺增生症（BPH）的安全性和疗效.方法：选取年龄70～83岁的患者40例,于内科疾病治疗稳定后手术.所有患者均行经尿道前列腺电切术,术中确定精阜位置及到膀胱颈距离后进行手术.结果：在积极进行围手术期处理后,40例患者均手术成功,无中转开放手术.患者术后恢复良好,国际前列腺症状评分由术前（22.5±6.0）分降至（7.1±1.8）分;生活质量评分由术前（4.4±1.5）分降至（1.7±0.5）分;残余尿量由术前（139.0＋41.2）ml降至（39.0±15.3）ml;最大尿流率由术前（7.9±0.9ml/s）升至（16.8＋4.1）ml/s.结论：TURP对高龄BPH患者来说,是一种安全有效、创伤小、恢复快的治疗方法.

Objective To investigate the curative effect of transurethral resection of prostate （TRUP） for treatment of elderly be- nign prostatic hyperplasia patients. Methods Clinical data of 40 benign prostatic hyperplasia patients with medical complications aging from 70 ~ 83 years were retrospectively analyzed. Operations were taken after their complications were controlled. Operations were star- ted after positions of seminal colliculus and distances to neck of bladder were confirmed. Results With individualized pefi - operational management, all operation were performed smoothly without any complications. Complications. The international prostste symptom score （IPSS） decreased from （22. 5 ＋6.0） before operation to （7.1 ~ 1.8） after operation. Quality of life （QQL） decreased from （4.4 ＋ I. 5） before operation to （ 1.7 ~ 0. 5） after operation. Post - void residual urine （PVR） decreased from （ 139. 0 ~ 41. 2） ml before operation to （39. 0 ~ 15. 3） ml after operation. Maximum flow rate （Qmax） increased from （7. 9 s0. 9） ml/s before operation to （16. 8 ~4. 1 ） ml/s after operation. Conclusions TURP is a safe and effective method with less trauma and fast recovery for treating elderly benign prostatic hyperplasia patients.
