摘要
目的探讨膜式薄层液基细胞学技术(TCT)在支气管肺泡灌洗液细胞学制片中的应用价值。方法收集2010年1月至2012年8月所有支气管肺泡灌洗液标本共计238例,同时采用传统离心沉淀涂片和膜式薄层细胞学检测系统进行细胞学检查,对结果进行统计分析,数据采用Vilcoxon秩和检验。结果 TCT制片细胞结构及背景清晰,细胞单层分布,无重叠;细胞量优于传统离心沉淀涂片,TCT法的阳检率和优片率均高于传统离心沉淀涂片,差异有显著性(P<0.005)。结论 TCT技术在支气管肺泡灌洗液细胞学制片上明显优于传统涂片方法,能提高制片中的细胞量及阳检率,从而减少复检次数,为临床提供准确、及时的诊断依据。
Objective To explore the significance of application of thin liquid based cytologic test ( thinprep cytologic test, TCT) in cy tologic examination of bronchoalveolar lavage fluid. Methods A total of 238 bronchoalveolar lavage specimens in this hospital during January 2010 to August 2012 were collected, while the traditional centrifugal sedimentation smears and membrane type thin layer cytologic detection system had been applied for cytologic examination, the quality of these smears had been evaluated and their results were analyzed with chi square test. Results The cell structure and background were distinctly clear, all cells on TCT smears were distributed as monolayer, without overlapping; and the amount of cells were superior to that of traditional centrifugal precipitation smears (CP). The difference in positive rate between these two methods had statistical significance ( P 〈 0.05 ). Conclusion The application of TCT technique in preparation of cytologic smears with bron choalveolar lavage fluid is obviously superior to that with traditional method, it can raise the amount of cells and positive rate, therefore it may re duce the time for reexamination and provide accurate and prompt diagnosis for clinicians.
