摘要
目的观察白内障超声乳化术后人工晶状体眼及正常对照眼各自的高阶像差内部相关性,探讨关键的分阶像差及其临床意义。方法采用Zywave波前像差仪,检测129例(129眼)白内障超声乳化术后人工晶状体眼组(试验组)与107例(107眼)正常对照组眼(对照组)的眼总高阶像差、球差(Z400)、彗差(Z310,Z311)等各分阶像差均方根值(RMS),对于组内总高阶像差与各分阶像差的关系采用偏相关分析。结果经偏相关分析控制高阶像差内部其他分阶像差的可能相互作用,在试验组中,Z310、Z441、Z421、Z400、Z420、Z440、Z530与总高阶像差具有相关性(偏相关系数分别为0.25、-0.24、0.19、-0.33、0.32、0.20、-0.34;均为P<0.05);在对照组中,Z331、Z311、Z400、Z551与总高阶像差具有相关性(偏相关系数分别为0.21、-0.28、-0.42、-0.32;均为P<0.05)。无论手术与否,只有球差(Z400)与总高阶像差始终保持了相关性且相关性较高(试验组和对照组中的偏相关系数分别为-0.33和-0.42)。结论白内障术后人工晶状体眼与正常对照眼中,在眼高阶像差的内部影响中,球差(Z400)与总高阶像差的关系最为密切,是影响总高阶像差的最主要的分阶像差。
Objective To observe the internal correlation of high-order aberrations of pseudophakic eyes after phacoemulsification surgery and normal eyes, and investigate the key aberrations and their clinical significances. Methods By Zywave wavefront instrument, the high-order aberrations such as general high-order aberrations, spherical aberration, and coma were detected for their root mean squares (RMS) of pseudophakic group (129 eyes) and normal group (107 eyes). The correlation of general high-order aberration and partial high-order aberrations of each group was com- mitted by partial correlation analysis. Results Controlling the possible interaction of the partial high-order aberrations by partial correlation analysis, in pseudophakic group, the correlation between general high-order aberrations and Z310, Z441, Z421, Z400, Z420, Z440, Z530 were statistically significant (partial correlation coeffmient R were 0.25, -0. 24,0.19, -0.33,0.32,0.20, -0.34 ;All P 〈 0.05 ). In normal group, the correlation between general high-order aberrations and Z331, Z311, Z400, Z551 were statistically sig- nificant (partial correlation coefficient R were 0.21, -0.28, -0.42, -0.32;All P 〈 0. 05). Whether the operation or not, only spherical aberration (Z400) always maintained correlation with general high-order aberration (partial correlation coefficient were - 0.33, - 0.42 in two groups). Conclusion Of the aberrations in the pseuphakic and control groups, spherical aberration (Z400) has the closest correlation with general high-order aberration and is the most primary high-order aberration.
