摘要
目的探讨酶联免疫斑点技术(ELISPOT)在淋巴结核病快速诊断中的临床应用价值。方法利用结核菌特异抗原EAST-6和CFP-10,通过ELISPOT技术,分别检测61例淋巴结核病患者、35例健康体检者和30例非结核淋巴结病患者外周血结核效应T淋巴细胞的数量,并与抗酸染色、PPD试验、结核抗体等方法进行比较。结果 61例淋巴结核病患者中,49例结核抗原特异ELISPOT阳性,35例健康体检者中,1例阳性,30例非结核淋巴结病患者中,仅1例阳性,提示该方法的敏感度、特异度分别为80.3%和96.7%。ELISPOT方法检测淋巴结核病患者的阳性率为80.3%,显著高于抗酸染色、PPD试验和结核抗体的检测。结论 ELISPOT技术是一种灵敏度高、特异性好的快速检测结核菌感染的方法,可用于淋巴结核病的快速诊断。
Objective To explore the clinical application value of the enzyme -linked immunospot assay (ELISPOT) in rapid diagnosis of lymphoid tuberculosis. Methods Sixty -one patients with lymphoid tuberculosis, 35 healthy person and 30 patients with non - tuberculosis lymphadenopathy were enrolled in this study. The peripheral blood T lymphocytes related to tuberculosis were detected by ELISPOT, using the mycobacterium tuberculosis - specific antigen EAST - 6 and CFP - 10, and compared with anti - acid dyeing, PPD test and tuberculosis antibody test. Results Among the 61 lymphoid tuberculosis patients, 49 eases were tuberculosis antigen positive by specific ELISPOT, and 1 case was positive among the 35 healthy person and 30 patients with non - tuberculosis Iymphadenopathy respectively. It suggested that the sensitivity and specificity of the ELISPOT were 80.3% and 96.7% respectively. And the positive rate was 80.3% in the diagnosis of lymphatic tuberculosis by ELISPOT, significantly higher than anti - acid dyeing, PPD test and tuberculosis antibody test. Conclusion ELISPOT is high sensitive and specific in rapid detection of mycobacterium tuberculosis infection, and it can be used for rapid diagnosis of lymphoid tuberculosis.
