激素替代治疗周期402例冻融胚胎移植的回顾性分析 被引量:1

402 cases of frozen-thawed embryo transfer in hormone replacement therapy cycles: a retrospective study

摘要 目的回顾性分析激素替代治疗(hormone replacement therapy, HRT)周期冻融胚胎移植(frozen - thawed embryo transfer, FET)的妊娠结局并探讨其影响因素。方法回顾性分析2011年1月~2012年1月中山大学附属第一医院生殖中心接受激素替代治疗周期冻融胚胎移植的不孕患者的妊娠结局和其相关因素。结果研究期间366个患者行402个HRT周期冻融胚胎移植,临床妊娠率为41.5%,年龄≤35岁患者妊娠率为43.4%;〉35岁患者妊娠率为27.7%。多胎率为29.9%,种植率为22.4%,其中D3天胚胎种植率为19.7%,囊胚种植率为37.7%。接受单个胚胎移植的患者,D3天胚胎种植率为9.1%,囊胚种植率为53.3%。结论激素替代周期冻融胚胎移植能获得满意种植率,影响其临床妊娠率因素包括女方年龄、ET日雌激素水平,以及ET胚胎数目、类型和质量。 Objective : To review the pregnancy outcome and the affecting factors of frozen - thawed embryo transfer in hormone replacement therapy cycles. Method : Retrospectively reviewed the pregnancy outcome and the affecting factors of subfertile patients undergoing frozen -thawed embryo transfer in hormone replacementtherapy cycles between January 2011 and January 2012 in our IVF center. Results: A total of 402 frozen -thawed embryo transfer cycles performed in 366 patients during the study period were analyzed. The overall clinical pregnancy rate was 41.5%, while the rates in younger (≤35 years ) and older ( 〉 35 years) infertile women were 43.4% and 27. 7%, respectively. The overall multiple pregnancy rate was 29. 9% and the implantation rate was 22. 4%. Cleavage stage embryo ( D3 post - insemination) transfer resulted in an implantation rate of 19, 7% and blastoeyst - stage embryo ( D5 post - insemination) reached a higher implantation rate of 37. 7%. However, in single embryo transfer cycles, the implantation rates of cleavage stage embryo and blastoeyst - stage embryo were 9. 1% and 53.3%, respectively. Conclusions: Clinical pregnancy rate of frozen -thawed embryo transfer in HRT cycles was 41.5%. Higher pregnancy rates were associated with younger maternal age (≤35 years) and blastocyst- stage embryo transfer.
