门诊药房处方调配差错原因分析及预防措施

Influencing factors analysis and c preventive measures of outpatient prescription deployment errors

目的：探讨门诊处方调配出现差错的原因以及相应的预防措施。方法：搜集2011年5月～2013年5月间我院门诊药房出现调配差错的处方并进行回顾性分析，总结发生差错的原因，并制订相应的预防对策。结果：两年间，我院门诊药房共发生处方调配差错94例，导致发生差错的因素依次为：医师因素、药师因素和其他因素。结论：在医院门诊药房加强针对性管理、规范处方调配规程、提高药学服务人员业务水平，是预防差错出现的有效措施。

Objective：To observe the influencing factors and preventive measures of outpatient prescription deployment errors. Methods： The data of outpatient prescription deployment errors in our hospital from May 2011 to May 2013 were collected and analyzed retrospectively. The influencing factors and preventive measures were discussed. Results：The total error cases of outpatient prescription deployment errors in our hospital were 94 in the two years. The influencing factors were physician factor, pharmacist＇s factor and other factors. Conclusion： In outpatient pharmacy, strengthening management, improving the professional quality of the pharmacy pharmacists can effectively prevent prescription deployment error occurred.
