社区医院门诊抗菌药物使用横断面调查

Cross-sectional study on use of antibiotics in outpatient department of community hospital

目的调查某社区医院中心门诊部抗菌药物临床应用情况,以促进医院抗菌药物的合理应用。方法采用横断面调查方法,随机对医院中心门诊部2012年2月1619张、5月1622张电子处方进行分析,并以药物应用指数(DUI)及2011年卫生部全国抗菌药物临床应用专项整治活动方案进行合理用药评价。结果门诊抗菌药物处方使用率分别为2月份46.45%、成人41.09%、儿童72.30%;5月份处方率37.61%、成人35.43%、儿童50.21%;成年及儿童抗菌药物应用情况均显示5月份比例低于2月;两次横断面抗菌药物应用情况调查显示,多数科室5月份低于2月份;成人主要抗菌药物针剂使用分析显示2月份与5月份的DUI大致相似;抗菌药物使用级别以非限制类抗菌药物为主,无特殊级别抗菌药物使用。结论两次横断面调查显示医院抗菌药物使用基本合理,5月份较2月份有明显改观,但尚需加强管理。

OBJECTIVE To investigate the clinical application of antibiotics in central outpatient department of community hospital so as to promote the rational use of antibiotics. METHODS By means of cross-sectional investigation, a total of 1619 pieces of prescriptions and a total of 1622 pieces of electronic prescriptions which were randomly enrolled in Feb and May 2012 were analyzed, then the rational use of antibiotics was evaluated by referring to drug utilization index （DUD and the nationwide special project control scheme of clinical application of antibiotics by the Ministry of Health, 2011, RESULTS The utilization rate of antibiotics prescriptions of outpatient department was 46.45% in Feb, 41.9% of which were for the adults, 72.30% for the children; the utilization rate of the antibiotics prescriptions was 37.61% in May, 35.43% of which were for the adults, 50.21 were for the children, indicating that the proportions of both the adults and the children who used antibiotics were lower in May than in Feb; the cross-sectional survey indicated that the proportion of the antibiotics prescriptions of most of the department was lower in May than in Feb; the analysis of the antibiotics application of the adults revealed that the DUI in Feb was similar to that in May; the use of non-restricted antibiotics was the major type of medication, and no special antibiotics were used. CONCLUSION The two cross-sectional investigations have revealed that the use of antibiotics had been basically reasonable and had been significantly improved in May as compared with that in Feb, but it remains to be further enhanced.
