乌司他丁对右肺癌切除患者术中炎性因子的影响 被引量:2

Effects of Ulinastatin on Inflammatory Cytokines in Patients Undergoing Resection for Right Lung Cancer

摘要 目的探讨乌司他丁对右肺癌切除患者术中炎性因子的影响。方法选择28例右肺癌手术患者,按随机数字表法分为乌司他丁组(n=14)和对照组(n=14)。乌司他丁组于开放静脉后将乌司他丁1万U·kg-1稀释成50mL,30min泵入;对照组给予等量生理盐水。于中心静脉置管时(T1)、开胸时(即单肺通气时,T2)、机械通气结束时(T3)经中心静脉导管各抽取静脉血3mL。采用ELISA双抗体夹心法检测血清中TNF-α、IL-6、IL-8水平,并观察其在不同时点的变化。结果与T1时点比较,在T3时点对照组血清TNF-α水平显著升高(P<0.01)。在T3时点,血清TNF-α、IL-6、IL-8水平乌司他丁组较对照组明显降低(均P<0.05);在T1、T2时点,2组血清TNF-α、IL-6、IL-8水平比较差异无统计学意义(均P>0.05)。结论乌司他丁可减轻肺癌手术患者肺泡炎性反应,对其有保护作用。 Objective To investigate the effects of ulinastatin(UTI)on inflammatory cytokines in patients undergoing resection for right lung cancer.Methods Twenty-eight right lung cancer patients scheduled for pneumonectomy were randomly divided into two groups,with 14patients in each group.The UTI group was given intravenous injection of 10 000U·kg-1(ulinastatin was diluted into 50mL and infusion time was 30minutes).The control group was given the same volume of saline.Venous blood samples(3mL)were taken from internal jugular vein at the beginning of central venous catheterization(T1),beginning of one-lung ventilation(T2),and end of mechanical ventilation(T3).Serum levels of TNF-α,IL-6and IL-8were measured by ELISA.Results In control group,the levels of TNF-αat T3were significantly higher than those at T1(P&lt;0.01).At T3,the levels of TNF-α,IL-6and IL-8in UTI group were significantly lower than those in control group(P&lt;0.05).At T1and T2,the differences in levels of TNF-α,IL-6and IL8were not obvious between the two groups(P&gt;0.05).Conclusion UTI can alleviate alveolar inflammatory response in patients undergoing lung cancer surgery.
