超声引导下腹横肌平面阻滞在全子宫切除术+盆腔淋巴结清扫术后镇痛的应用 被引量:8

摘要 目的观察超声引导下腹横肌平面阻滞应用于开腹广泛全子宫切除术+盆腔淋巴结清扫术后镇痛的效果。方法选择拟行开腹广泛全子宫切除术+盆腔淋巴结清扫术患者60例,随机分成术后镇痛腹横肌平面阻滞(TAPB)+患者自控静脉镇痛(PCIA)(观察组)及单纯PCIA组(对照组),每组各30例。记录两组患者术后清醒拔管即刻(T0)、2h(T1)、6h(T2)、12h(T3)、24h(T4)各时间点的HR、血压、呼吸频率、SpO2、静息及运动VAS视觉模拟评分、Ramsay镇静评分;记录两组患者第1次需求自控镇痛(PCA)距离清醒拔管的时间(T),24h镇痛泵药物使用量(V),24h PCA有效按压次数(D1)、PCA实际按压次数(D2)及按压有效率(D1/D2);记录两组不良反应及术后镇痛的满意度评分。结果对照组患者术后各时间点的静息及运动VAS评分均高于观察组;Ramsay评分在T0和T1时点均低于观察组(P<0.05)。对照组患者第一次需求PCA距离清醒拔管的时间(T)明显小于观察组(P<0.01);24h镇痛泵药物使用量(V),PCA有效按压次数(D1)及PCA实际按压次数(D2)均高于观察组(P<0.01);对照组24h按压有效率(D1/D2)低于观察组;呕吐的发生率明显高于观察组;术后镇痛的满意度低于观察组(P<0.01);结论超声引导下腹横肌平面阻滞减少了开腹广泛全子宫切除术+盆腔淋巴结清扫术患者术后镇痛的阿片类药物用量及副作用,增强了该类手术患者的术后镇痛效应。
