脑梗死患者血浆同型半胱氨酸检测的临床作用

摘要 目的探讨临床对脑梗死患者行血浆Hcy(同型半胱氨酸)检测的作用。方法神经内科接收130例脑梗死患者,称为临床检测组。抽选健康体检者130例为对照研究人员,称为正常体检组。在入院次日且空腹12h时,于肘静脉采集血液5ml。经FPLA(荧光偏振免疫)检测Hcy。结果在临床检测组(130例)中,发生高Hcy的概率、Hcy水平都比正常体检组(130例)高,P<0.05。临床检测组的Hcy水平和治疗前对比,下降明显,P<0.05。倘若病情呈现加重趋势,Hcy水平就会上升,神经功能缺损显示分值会增高。各亚组(重型脑卒中、中型脑卒中、轻型脑卒中)间差异显著,P<0.05。结论脑梗死患者显示Hcy水平明显要比健康人群高,病情轻重和Hcy水平高低成正比,有效的治疗可使Hcy水平明显下降,Hcy水平是疗效观察、临床诊断的参考指标,临床作用很大。 Objective To investigate the clinical function of plasma homocysteine (Hcy) test in patients with cerebral infarction. Methods A total of 130 patients with cerebral infarction admitted to the department of neurology were selected as clinical test group, while 130 healthy people who underwent physical examination were selected as normal control group. Blood (5 ml) was collected from each person via the cubital vein after 12 h of fasting in the second day of hospitalization. Fluorescence polarization immunoassay was adopted for Hcy measurement. Results The clinical test group had significantly higher incidence of high Hcy and Hcy level than the normal control group (P〈0.05). The clinical test group showed significantly decreased Hey level after treatment (P〈0.05). With the worsening of the condition, Hcy level and neurological deficit scores would increase accordingly. There were significant differences between subgroups (severe cerebral stroke, moderate cerebral stroke, and mild cerebral stroke) (P〈0.05). Conclusion The Hcy level in patients with cerebral infarction is significantly higher than that in healthy people. The severity of cerebral infarction is proportional to Hcy level, which can be remarkably decreased by effective therapy. Hcy level, which is of great importance, is a reference index for curative effect observation and clinical diagnosis.
