右美托咪定对全凭静脉麻醉单肺通气时氧合指数的影响 被引量:5

摘要 右美托咪定是一种新型的肾上腺素能α2受体激动剂,与α2受体的结合能力是可乐定的8倍,具有镇静、催眠和镇痛等作用且不影响呼吸,因此目前广泛应用于重症监护室和围手术期。单肺通气是许多胸外科手术中必不可少的环节,单肺通气时,非通气肺有血流而无通气即出现从右到左的肺内分流,导致动脉PaO2显著下降,有5%-10%的患者会出现低氧血症[1],目前已知有许多因素可影响 PaO2,包括缺氧性肺血管收缩(HPV)、手术体位、双腔管位置以及通气策略。单肺通气期间出现低氧血症,主要原因是HPV效应减弱引起肺内分流量过大[2]。右美托咪定对单肺通气时的氧合指数的影响尚不明确,由于吸入麻醉药会减弱HPV效应,笔者通过探讨全凭静脉麻醉下右美托咪定对单肺通气的氧合指数影响,旨在为临床应用提供参考,现报道如下。
