直肠癌切除术术中盆底腹膜重建36例分析

Analysis of 36 cases of rectal cancer resection of pelvic floor reconstruction surgery

目的：探讨直肠癌Miles手术和Dixon手术术中直接关闭重建盆底腹膜的临床效果。方法：回顾性分析我科36例低位直肠癌Miles手术和Dixon手术患者盆底腹膜重建手术的临床疗效，观察患者输尿管狭窄、盆底疝等相关术后并发症，以及术后复发姑息性放疗准确定位。结果：36例患者中盆底小肠疝1例，输尿管梗阻1例，术后5年后复发6例，术后6个月复查MRI与术前相比较，盆底有所下降。结论：直肠癌Miles手术和Dixon手术盆底腹膜重建术后并发症少，一旦复发，能为姑息性放疗定位做好解剖基础，对临床系统治疗直肠癌意义重大。

Objective To explore the efficacy of surgical treatment of Carcinoma of the rectum by rehabilitating the peritoneum of under--pelvis with the method of Miles and Dixon. Methods To assay the efficacy of treating 36 patients of Carcinoma of the lower rectum by rehabilitating the .peritoneum of under-pelvis with the method of Miles and Dixon, Observing the complication after operation, such as Ureterostegnosis, hernia of under-pelvis and correcting ori-entation for over-tolerating radiotherapy of recurrence after operation. Results There is 1 patient occurring hernia of under-pelvis, 1 patient occurring Uret- erostegnosis, there are 6 patients of reeurrence of all Under-pelvis is lower after operation for six months from MRI of after-operation and before-opera-tion. Conclusion There are little complication of surgical treatment of Carcinoma of the rectum by rehabilitating the peritoneum of under-pelvis with the method of Miles and Dixon,Once recurrence,can be the base of dissection for over-tolerating radiotherapy of recurrence after operation. It is important effi-cacy to treat Carcinoma of the rectum by surgical systematicness.
