带锁髓内钉在股骨干骨折中的临床应用

Clinical application in treating femoral shaft fracture intramedullary interlocking nail

目的：观察带锁髓内钉穿钉固定治疗股骨干骨折的疗效。方法：2009年1月-2010年1月我们使用带锁髓内钉内固定股骨干骨折30例，回顾性分析此术式的治疗效果，总结术中注意事项。结果：本组患者骨折愈合时间平均18．5周，骨折愈合后1～2年取钉。下肢功能良好，无畸形愈合，无术后感染和断钉，无神经和血管损伤、股骨颈骨折等并发症。自定评定标准：无疼痛，步态正常，髋膝关节功能正常，评定为优。优21例，良8例，可1例，骨折愈合优良率为92．5％。结论：带锁髓内钉开放性穿钉内固定避免了医护人员和患者的X线熙射，治疗优良率较高，是治疗股骨干骨折的较好方法，带锁髓内钉在复杂的粉碎性骨折中的临床应用，扩大了手术适应证，具有创伤小、固定牢靠、能早期功能锻炼、减少并发症、疗效好等效果，值得基层医院临床推广。

Objective： To observe the effects of interlocking intramedullary nail nail fixation for treatment of femoral shaft fracture. Methods：2009 January - 2010 January we used interlocking intramedullary nailing of femoral shaft fractures in 30 cases, a retrospective analysis of the treatment effect, matters needing attention in operation were summarized. Results：in this group, patients with fracture healing time was 18. 5 weeks in average, fracture healing after 1-2 nail. Lower limb function good, malunion, and broken nails without postoperative infection, no nerve and vessel injury, femoral neck fractures. Self evaluation：no pain, normal gait, hip and knee function, assessed as excellent. Excellent in 21 cases, good in 8 cases, 1 cases, the excellent and good rate was 92.5% in fracture healing. Conclusion：intramedullary nail with lock open nail fixation to avoid the exposure of the medical staff and patients, the excellent and good rate was higher in treatment, is a good method for the treatment of femoral shaft fracture, interlocking intramedullary nail crushed in the clinical application of fracture in the complex, expanded the operation indications, with small trauma, firm fixation, early functional exercise, can reduce the complications, efficacy and good effect, worthy of clinical popularization in primary hospital.
