急性颅脑损伤的CT颅内阳性并发症与临床特征相关性研究 被引量:2

Preliminary Study of the Interdependency Between the Positive CT Findings of Intracalvarium and Clinical Characteristics of Acute Head Injury

摘要 目的分析急性颅脑损伤的CT颅内阳性并发症与临床特征的相关性,探讨急性颅内损伤的临床危险因素及CT检查的适应症。方法对230例急性颅脑损伤患者的颅脑CT表现与临床特征进行前瞻性研究。以临床特征作为自变量,CT显示的急性颅脑损伤作为因变量进行多因素Logistic回归分析。结果与所有颅内损伤相关性较强的临床危险因素依次为神经查体阳性(OR=3.05)、危险的外伤机制(OR=2.37)。对严重的颅内损伤,相关性较强的临床因素依次为意识丧失(OR=4.13)、颅骨骨折征象(OR=3.85)。结论根据急性颅脑损伤出现颅内并发症的危险因素,可以初步确定CT检查的适应症,并预测急性颅内损伤的发生及损伤程度,在及时对危重病人进行检查的同时可减少不必要的CT检查。 Objective To analyse the interdependency between the positive CT findings of intracalvarium and clinical characteristics of acute head injury ,and explore the dangerous factors of acute intracranial injuries and the indication of CT. Methods The CT findings and clinical characteristics of acute head injury of 230 cases were studied prospectively. The clin ical findings as independent variable, the positive findings on CT as dependent variable, their association with multivariate a nalysis of Logistic Regression was tested. Results The associated clinical characteristics with all positive CT result were neu rological deficit (OR = 3.05), serious injury( OR = 2.37). The associated clinical characteristics with serous positive CT re sult were loss of consciousness ( OR = 4.13 ) , fracture of skull( OR = 3.85 ). Conclusion We have concluded that the pre liminary indications of CT for acute head injury according to the dangerous eharaeteristieal factors can help clinicians to evalu ate the occurrence and severity of acute intracranial injury correctly, and reduce the unnecessary CT.
