硬膜外吗啡复合静脉镇痛用于剖宫产术后镇痛的临床观察

Clinical observation on epidural morphine combined with intravenous postoperative analgesia for pain relief following cesarean section

目的探讨硬膜外小剂量吗啡复合静脉镇痛在剖宫产术后镇痛的优缺点。方法将180例剖宫产产妇根据术后镇痛方式随机分为:A(硬膜外)、B(静脉)和C(硬膜外吗啡+静脉)组。观察术后镇痛VAS评分和并发症,并进行产妇满意度调查。结果 C组VAS评分与A组无显著性差异;三组都没有呼吸抑制发生;C组产妇满意度(53例)明显高于A组(41例)和B组(44例)。结论硬膜外吗啡复合静脉镇痛是一种简单有效安全的剖宫产术后镇痛方式。

Objective To discuss the advantages and disadvantages of small dose of epidural morphine combined with intravenous postoperative analgesia for pain relief following cesarean section. Method 180 women undergoing elec- tive cesarean section were allocated randomly into three groups according to postoperative analgesia setting： A （epi- dural） ,B（ intravenous） ,C （epidural morphine＋ intravenous ）. VAS was used to evaluate the analgesic efficacy. Side effects like respiratory depression, excessive sedation, pruritus and nausea were recorded. Survey on degree of satisfaction was performed after analgesia. Result There is no statistic difference between group C and group A in VAS. No respiratory, depression in all the three groups. Group C has higher satisfaction than group A and group B. Conclusion Epidural morphine combined with intravenous Dstoperative analgesia is a simple, valid, and secure setting for pain relief following cesarean section.
