摘要
目的探讨人乳头状瘤病毒(HPV)基因分型检测在宫颈疾病诊疗中的分布特点,为临床应用提供参考。方法采用凯普医用核酸分子快速导流杂交基因分型试剂盒(HybriMax)对21种HPV基因型别进行分型检测,并对各种亚型的分布特点进行统计分析。结果在521例标本中共检出245例HPV阳性,21种型别的HPV均被检出,HPV感染率为47.02%;感染率最高的型别是HPV16,为11.90%,最低的是HPV43,为0.19%,其他型别以感染率从高到低依次为HPV58、52、6、11、18、CP8304、53、68、56、39、31、33、66、59、42、51、45、44、35;其中检出HPV高危基因型感染患者208例,感染率前3位的依次为16、58、52,HPV低危基因型感染患者80例,阳性率依次为6、11、42、44和43;521例HPV阳性样本中,单一型感染为140例,占26.87%,其中高危亚型105例,占20.15%,低危亚型35例,占6.72%;双重感染共为68例,占13.05%;多重感染(低、高危两型及两型以上)共为37例,占7.10%。结论该地区人群中存在较高的HPV感染率,进行HPV基因分型筛查对宫颈疾病早期发现、判断预后及指导治疗有重要的临床价值。
OBJECTIVE To explore the genotypes of human papilloma virus (HPV) causing cervical diseases so as to guide the clinical treatment. METHODS Totally 21 genotypes of HPV were genotyped and identified by using the Cape medical nucleic acid hybridization quick diversion genotyping kit (HybriMax)method, then the distribution of the subtypes was statistically analyzed. RESULTS Of totally 521 specimens, 245 were tested positive for HPV, and all the 21 genotypes have been detected, the incidence rate of HPV infection was 47.02%, the incidence rate of HPV 16 infections was the highest (11.90%), the HPV43 infection was the lowest (0.19%), and the incidence rates of the other genotypes of HPV infection from high to low were as follows:the HPV58, 52, 6, 11, 18, CP8304, 53, 68, 56, 39, 31, 33, 66, 59, 42, 51, 45, 44, and 35% there were 208 cases of patients diagnosed as high risk HPV infection, the incidence rates of the HPV16 infection, HPV58 infection, and HPV52 infection ranked the top three% thee were 80 cases of patients diagnosed as the low risk HPV infection, the positive rates were respectively 6, 11, 42, 44, and 43. Among the 521 cases of patients with HPV tested positive, there were 140 (26.87%) cases of single genotype HPV infection, including 105 (20. 15%) cases of high risk HPV infections and 35 (6.72%) cases of low risk HPV infection, and there were 68 (13.05%) cases of dual infections, including 37 (7.10%) cases of dual infections(low risk , high risk, and both) CONCLUSION The incidence of HPV infections is high among the population in this area% the screening of the HPV genotypes has great clinical significance in the early identification of cervical diseases, and judgment of prognosis, as well as guidance of treatment.
