胸外按压与人工通气比之窘境 被引量:25

摘要 自从1958年Peter Safar等创造了人工呼吸、1960年Kouwenhoven等[2]报道徒手胸外按压术以来,现代心肺复苏(CPR)中胸外按压与通气的比例经历了从5:1和15:2,直至2005年《国际心肺复苏指南》将按压通气比例调整为30:2[3]的过程,其目的在于通过增大胸外按压的比例,为重要器官提供有效的血流灌注;然而不论比例如何变更,CPR成功率仅有5% ~ 10%,并没有显著提高.追其原因不得不从CPR的源头上考量,尤其是现行的按压与通气不能同步进行,即胸外按压时只有循环而无通气,而后予以人工通气时又无人工循环维系,导致通气/血流比例失调,肺内换气不能有效地进行,必将影响CPR的质量.故以往按压与通气比例的变更仅仅是一种"量"的调整,并未从按压与通气有机同步进行的"质"上变化,如何走出胸外按压与通气比之窘境,创建持续人工循环状态下给予人工通气的新模式,是当今吾辈CPR工作者必须承担的历史使命.
