摘要
目的利用多层螺旋CT研究主-髂动脉分叉部的解剖学特点。方法回顾分析49例腹部及盆腔CT增强扫描资料,使用容积再现(VR)图像,观察主-髂动脉分叉部的位置,并测量左、右侧髂总动脉与腹主动脉在冠状面的夹角及矢状面的倾斜角,分析其在性别间及侧别间是否存在统计学差异。结果主-髂动脉分叉部位置以L4椎体中1/3份水平及下1/3份水平为主,占55.1%。冠状面髂总动脉与腹主动脉夹角,右侧男性为(21.70±7.19)°,女性为(23.66±5.64)°;左侧男性为(16.44±6.23)°,女性为(19.68±6.75)°。髂总动脉与腹主动脉夹角右侧性别间无统计学差异(P=0.134);左侧性别间有统计学差异(P=0.012);男性、女性在侧别间均有统计学差异(P=0.006,P=0.024)。矢状面髂总动脉倾斜角右侧男性为(10.42±4.59)°,女性为(11.45±5.93)°;左侧男性为(11.96±6.66)°,女性为(16.75±8.55)°。髂总动脉倾斜角左、右侧性别间均无统计学差异(P=0.565,P=0.070);男性侧别间无统计学差异(P=0.438);女性侧别间有统计学差异(P=0.029)。结论多层螺旋CT能为临床提供主-髂动脉分叉部的解剖信息,对血管外科介入治疗具有指导意义。
Objective To discuss anatomy characteristics of aortoiliac artery bifurcation by multislice spiral CT. Meth ods Volume rendering (VR) images of 49 patients with abdomen and pelvic enhanced spiral CT scan were reviewed. The position of aortoiliac artery bifurcation, and the angle between the both sides of common iliac artery and the abdominal aorta in coronal plane and the tilt angle in sagittal plane were observed. The data of both sides and sex for any significant differ- ences were analyzed. Results The main positions of aortoiliac artery bifurcation were low and medium one third of L4 (55.1%). In coronal plane, the angle between the right common iliac artery and abdominal aorta were (21.70 ±7.19)° in male ; (23.66 ±5.64)° in female ; the left angle were ( 16.44 ± 6.23 ) ° in male ; ( 19.68 ± 6.75 )° in female. There was no gender differences (P =0. 134) in the right angle, there were gender differences (P =0. 012) in the left angle. There were statistically differences (P = 0. 006, P = 0.024) between the two sides in male and female. In sagittal plane, the right side tilt angle in male were (10.42 ±4.59)°, (11.45±5.93)°in female;the left side tilt angle were (11.96 ± 6.66) in male, ( 16.75 ± 8.55)° There was no gender differences (P = 0. 565, P = 0.070) in the right and left tilt angle, and also no statistically differences ( P = 0. 438 ) between the two sides in male. Conclusion Multislice spiral CT can provide information about anatomy of aortoiliac artery bifurcation for clinical practice, and has clinical significance for endovascular interventional treatment.
