管状胃对食管癌根治术后患者预后的影响

The Prognosis Study in Patients with Gastric Tube Reconstruction after Radical Esophagectomy

[摘要]目的探讨管状胃代替食管对食管癌根治术后患者生活质量的影响。方法设计我院2012年6月～2013年3月行食管癌根治术患者135例，随机分为实验组（管状胃替代食管，63例）与对照组（全胃替代食管，72例），比较两组患者手术情况、术后并发症发生率、生活质量相关指标。结果入组患者治疗均获成功，无术中死亡病例，术中相关指标比较，差异无显著性（P〉0．05）。术后并发症比较，实验组返流性食管炎与胸胃综合征的发生率明显低于对照组，差异有显著性（P〈0．05）。术后6周，两组患者生活质量比较无显著性差异（P〉0．05），实验组术后3个月以及术后6个月生活质量改善情况较对照组好，差异有显著性（P〈0．05）。结论管状胃治疗手段对于食管癌根治术后患者生活质量的改善有着积极的作用。

Objective To evaluate the quality of life in patients with esophageal cancer after the esophagecto- my followed by gastric tube reconstruction. Methods 135 patients were selected and randomly divided into two groups： gastric tube reconstruction group （63 patients） and whole stomach reconstruction group （72 patients）. The clinical features of the surgery, the incidence of complications and life quality of the two groups were compared re- spectively. Results All patients were survival without hospitalization death. There was no significant difference on the clinical features of the surgery between the two groups （P〉0.05）. The incidences of the reflux esophagitis and intrathoracic stomach syndrome of the gastric tube reconstruction group were significantly lower than that of the whole stomach reconstruction group （P 〈0.05）. In 6 weeks after the operation, the life quality of the two groups has no significantly difference （P 〉 0. 05）. In 3 months and 6 moths, the life quality of the gastric tube reconstruction group was significantly better than that of the whole stomach reconstruction group （P 〈0.05）. Conclusion The gastric tube reconstruction method is a positive way for the life quality of the patients.
