摘要
目的了解37—44周正常足月新生儿期睡眠-觉醒周期(SWC)发育的波幅整合脑电图(aEEG)特点。方法对入院的受孕龄(CA)37~44周无并发症新生儿行12~24h脑电监测,同时进行aEEG监测,分析aEEG宽带、窄带上下缘电压及带宽绝对值及以上各值随CA的变化特征。结果新生儿期至足月后期,随CA的增加窄带上缘逐渐降低,窄带下缘升高,带宽变窄,窄带上下缘电压及带宽值变化差异均有统计学意义;宽带上缘及下缘随CA增加呈逐渐增高,胎龄37~38周、38~40周、41~42周及43—44周组间差异有统计学意义;宽带带宽的变化差异无统计学意义。结论足月新生儿SWC的aEEG图形随CA的增加而变化。借助aEEG可以判断SWC是否存在;并同时判断患儿SWC图形与实际CA是否相符。
Objective To study the features of sleep - wake cycles ( SWC ) on amplitude - integrated electroen- cephalography (aEEG) of neonates of 37 to 44 weeks conceptional age. Methods The bedside EEG monitoring with duration of 12 -24 hours on CA37 -44W healthy neonates were performed. The aEEG was obtained synchronously. The amplitudes of upper and low edge voltage (UEV and LEV) , bandwidth values of narrow and broad bands Were recorded for correlation analysis with conceptional age. Results The UEV, LEV and bandwidth value of narrowband, and the UEV of broadband were significantly increased corresponding to increased conceptional age. However, no significant change of the LEV or bandwidth of broadband was observed with the increase of conceptional age. Conclusion The aEEG features of SWC of term neonate are various with the increasing CA. Clinicians can estimate the existence of SWC and its consistency with CA.
