脉图检测阿立哌唑和氟哌啶醇对精神分裂症患者心脏功能和血流动力学的影响 被引量:11

摘要 目的了解抗精神病药物对心脏功能和血流动力学的影响,更好地为临床用药提供依据。方法用随机数表的方法将102例患者分成阿立哌唑组53例,氟哌啶醇组49例。应用血液循环动力学脉图检测仪检测精神分裂症患者服用阿立哌唑和氟哌啶醇前后的血液循环动力学变化。结果在治疗效果上,阿立哌唑与氟哌啶醇总疗效相当;在治疗时出现的症状方面,氟哌啶醇组心血管系统异常症状发生例数比阿立哌唑组多;在血液循环动力学参数方面观察到阿立哌唑组用药前后有效循环血量(ECV)、心率(HR)、能量转换指数(MEC)、左室机械效率(LME)、心肌缺阈值(MIT)和心肌耗氧量(MVO)均无明显变化(P>0.05),而氟哌啶醇组用药前后ECV、HR、MEC、LME、MIT和MVO均有变化(P<0.05或P<0.01),且ECV、MEC、MIT降低和MVO的增高,表明了患者的心肌细胞有受损现象。结论阿立哌唑对心血管影响性较小,安全性较高。 Objective To explore the impact of antipsychotics (aripiprazole and haloperidol) on the cardiovascular sys- tem in patients with schizophrenia. Methods With stochastic indicator method 102 patients were randomly divided in- to the set Aripiprazole group of 53 cases, 49 cases of haloperidol group. One hundred and two patients with schizophrenia were recruited and diveded randomly into two groups treated with aripiprazole or haloperidol respective- ly. The clinical hemodynamie changes after treatment were detected by sphygmogram, including effective circulatory volume (ECV), myocardium energy rechange constant (MEC), left-ventricular maehanic coefficient (LME) and so on. Results There were no significant differences on efficacy between two groups (P 〉 0.05). However, patients treated with haloperidol had more side effects in cardiovascular system, including apparent reducing of ECV, MEC, MIT and definite increasing of MVO (P 〈 0.05 or P 〈 0.01 ), which suggested that patiets in this group had myocardial cell dam- age. There were no obvious hemodynamic changes in patients treated with aripiprazole (Ps 〉 0.05). Conclusions Arip- iprazole has few impact on the cardiovascular system and safe in treating patients with schizophrenia.
