阿托伐他汀对不稳定型心绞痛患者C反应蛋白和肿瘤坏死因子的影响

Clinical effect of atorvastatin on C-reactive protein and tumor necrosis factor of UAP patients

目的观察阿托伐他汀对不稳定型心绞痛（unstableanginapectoris，UAP）C反应蛋白和肿瘤坏死因子的影响。方法选择2010年1月-2013年1月在我院住院接受治疗的不稳定型心绞痛患者共80例．根据治疗方法不同随机分为观察组和对照组，各40例，两组均予抗凝、抗血小板聚集、硝酸酯类等药物治疗；观察组加用阿托伐他汀（立普妥，辉瑞公司生产）10mg口服，每晚顿服。疗程为4周。观察比较两组患者治疗后的疗效及两组不稳定型心绞痛患者治疗前后CRP、TNF-α的变化情况。结果两组患者均顺利完成治疗，无一例退出，其中观察组治疗后的总有效率达90．0％（36／40），对照组治疗后的总有效率达77．5％（31，40），两组比较差异具有显著性（x2=6．235，P〈0．05）。治疗前两组患者的CRP、TNF-α比较，差异元显著性；治疗4周后，两组患者的CRP、TNF-α均较治疗前明显降低，且观察组患者的CRP、TNF-α水平治疗后较对照组降低更显著，差异具有显著性（P〈0．05）。结论阿托伐他汀治疗不稳定型心绞痛疗效确切，考虑其可能是通过抑制炎症反应来减轻UAP患者的心肌损伤、改善UAP患者预后。

Objective To evaluate the clinical effet of atorvastatin in treatment of UAP,and to explore the impact on C-reactive protein and tumor necrosis factor. Methods Selected 80 cases of patients with unstable angina from Jan- uary 2010 to January 2013 in our hospital, 80 cases patients according to the different treatment methods were ran- domLy divided into observation group and control group 40 cases,two groups were treated anticoagulant, antiplatelet aggregation, nitrates and other drugs; observation group were treated with atorvastatin 10 mg orally every night,total 4 weeks. After treatment, the efficacy and the level of CRP, TNF-α changes were observed and compared. Results Two groups of patients were successfully completed treatment, the total effective rate of observation group was 90.0%（36/ 40）, the total effective rate of the control group was 77.5%（31/40）, the difference was significant （X2=-6.235 ,P〈0.05）. After treatment,the level of CRP, TNF-α of observation group and the control group was significantly lower than be- fore treatment,and the observation group decreased more significantly than the control group （P〈0.05）. Conclusion A- torvastatin in treatment of unstable angina has good efficacy, which may be considered by inhibiting the inflammatory re- sponse to relieve myocardial damage in patients with UAP and improve the prognosis of patients with UAP.
