超声对甲状腺癌和甲状腺腺瘤的诊断和鉴别诊断价值分析

目的探讨超声对甲状腺癌和甲状腺腺瘤的诊断和鉴别诊断价值。从而为甲状腺疾病的临床治疗提供有利的参考依据。方法回顾性分析我院2010年1月～2013年1月经手术病理证实的甲状腺癌患者26例及同期经手术后病理学证实的甲状腺腺瘤患者32例的临床及超声声像图资料。结果本组26例甲状腺癌中，经病理证实为乳头状癌21例，占80-8％，滤泡癌3例，髓样癌1例，混合癌1例。32例甲状腺腺瘤患者中，囊性变4例，合并非沙粒样钙化1例。位于左侧12例，右侧19例，峡部1例；双侧病变18例，单侧病变14例；甲状腺癌组Vmax与RI均明显高于甲状腺腺瘤组，差异具有显著性（P〈0．05）。26例甲状腺癌患者的血流分级以Ⅲ级为主。32例甲状腺腺瘤血流分级以I级为主。结论超声对甲状腺癌及甲状腺腺瘤的诊断和鉴别诊断具有重要的临床价值。

Objective To explore the diagnosis value of uhrasound of thyroid cancer and thyroid adenoma diagnosis and differential diagnosis, and thus for the clinical treatment of thyroid disease provide a favorable reference. Methods A retrospective analysis in our hospital from January 2010 to January 2013 pathologically confirmed 26 cases of thy- roid cancer, and pathology confirmed the same period after surgery in 32 patients with thyroid adenoma in patients with clinical and sonographic data. Results The group of 26 cases of thyroid cancer, papillary carcinoma confirmed by pathology in 21 cases, accounting for 80.8%, 3 cases of follicular carcinoma, medullary carcinoma, mixed carcinoma. 32 cases of patients with cystic thyroid adenoma, 4 cases with non sand-like calcification in 1 case. Located on the left 12 cases, 19 cases of fight isthmus one case; bilateral lesions in 18 cases, 14 cases of unilateral lesions; the Vmax and RI of thyroid cancer group ~vere significantly higher than thyroid adenoma group, the difference was significant （P 〈 0.05）. 26 cases of thyroid cancer in patients with class ]]I primary flow grade, 32 cases of thyroid adenoma flow grade to grade I based. Conclusion Ultrasound of thyroid cancer and thyroid adenoma diagnosis and differential di- agnosis has important clinical value.
