中心静脉导管在治疗恶性胸腔积液的护理策略

Nursing strategy of drainage malignant pleural effusion using a central venous catheter

目的探讨使用中心静脉导管在治疗恶性胸腔积液中的护理策略。方法 58例被确诊为恶性胸腔积液的患者均行胸腔闭式引流术,尽量引流尽胸水,注入顺铂治疗恶性胸腔积液,随机分为护理干预组(30例)和对照组(28例)。结果护理干预组发生胸闭管滑脱1例、局部渗液3例、合并肺部感染2例,明显优于对照组(P<0.05)。干预组发生发热及胃肠道反应明显减少,与对照组比较差异有统计学意义(P<0.05)。结论采用中心静脉导管对恶性胸腔积液患者进行胸闭引流,护理策略简单实用,可明显减少并发症,值得临床推广使用。

Objectives To explore nursing strategy of drainage malignant pleural effusion using a central venous catheter. Methods A total of 58 cases of malignant pleural effusion patients were accepted intermittent drainage of pleura1 effusion by central venous catheter and intrathoracically injected with cisplatin. All cases were randomly divid- ed into nursing intervention group （30 cases） and control group （28 cases）. Results One cases with catheter fall off, 3 cases local oozing and 2 cases pulmonary infection in nursing intervention group, were fewer than those in control group. The incidence of fever and gastrointestinal adverse reactions in nursing intervention group were significantly fewer than that of the control group （P〈0.05）. Conclusion The nursing strategy of drainage malignant pleural effusion using a central venous catheter is simply and practical. Its method can decrease complication and is worthy of promotion.
