摘要 目的探讨慢性化脓性中耳炎(CSOM)与感音神经性耳聋(SNHL)之间的相关性。方法回顾性分析单侧CSOM手术治疗64例患者的骨导阈值改变,分析0.5、1.0、2.0、4.0 kHz患耳与健耳骨导阈值的差异,单因素方差分析听骨链破坏对语频骨导阈值的影响。结果患耳与健耳各频率骨导阈值之间差异有统计学意义(P<0.05);听骨链受损组平均骨导阈值在2 kHz与4 kHz处>听骨链正常组(P<0.05)。结论 CSOM可引起感音神经性耳聋,高频骨导听阈较低频更易受到影响。听骨链损害与骨导听力下降有关,其共振作用的减弱或消失即可影响骨导听力的下降。 Objective To study the correlation between sensorineural hearing loss in patients with chronic suppurative otitis media. Methods This was a retrospective analysis of the clinical records and pure tone audiograms of patients with chronic suppurative otitis media (CSOM) in the hospital between Jan. 2009 and Jun. 2012 and 64 patients were studied. Results Chronic suppurative otitis media might lead greater bone conductive to suffer ears and cause sensorineural hearing loss. The mean bone conduction threshold differences between the diseased and control ears range across the frequencies 0.5, 1.0, 2.0, and 4.0 kHz. These differences tended to increase with increasing frequency and were all significant ( P 〈 0.05 ). The average bone conductive threshold of ossicular chain damage group greater than the normal ossicular chain group at the frequency (2,4 kHz) (P 〈 0.05 ). Conclusion Patients with CSOM have a significant degree of sensorineural hearing loss in this study. The higher frequencies are more affected. The damage of ossicular chain suffering ear can lead to bone increased conductive threshold.
