依匹斯汀联合他克莫司软膏治疗肛周湿疹疗效观察

Curative effects of epinastine combined with tacrolimus ointment on perianal eczema

目的观察依匹斯汀联合他克莫司软膏治疗肛周湿疹的临床疗效和不良反应。方法采用随机对照方法,将65例肛周湿疹患者随机分为两组,治疗组33例,予以口服依匹斯汀胶囊,1次/d,20 mg/次;外用0.1%他克莫司软膏,每日早晚各1次。对照组32例,仅外用他克莫司软膏,用法同治疗组。两组疗程均为4 w。分别于治疗后2、4 w时随访,观察皮损情况以及药物不良反应,比较两组疗效并记录不良反应。治疗结束后随访1个月,观察复发率。结果治疗组在治疗2、4 w时的有效率分别为69.70%和90.90%,优于对照组的43.75%和71.87%(P<0.05)。两组均未见严重不良反应;随访1个月后,治疗组复发率为3.33%,对照组复发率为26.09%,差异有统计学意义(P<0.05)。结论依匹斯汀联合0.1%他克莫司软膏治疗肛周湿疹疗效好,安全性高,复发率低。

Objective To observe the clinical curative effects of epinastine combined with tacrolimus ointment on perianal eczema and the adverse reaction. Methods A randomized control trial was adopted in this study. Sixty five patients were randomly divided into two groups. Treatment group （33 cases） orally took epinastine hydrochloride capsules once a day combing with external application of 0.1% tacrolimus ointment once every morning and night. Control group （32 cases） only externally applied tacrolimus ointment,and the usage was the same as the treatment group. The course of treatment was 4 weeks for both groups. Follow-up was made 2 and 4 hour after the treatment. The skin lesions and the drug adverse reaction were observed. The curative effects in both groups were compared, and the adverse reactions were recorded. The follow-up was made for 1 month after the treatment, and the observation was made in the relapse rate. Results The effective rates at the 2nd and 4th week in the treatment group were 69.70% and 90.90% ,respectively, which were higher than those in the control group （43.75% and 71.87% ,P 〈0.05）. Severe adverse reactions did not observed in the two groups. After the follow-up for 1 month,the recurrence rate in the treatment group was 3.33% ,and that in the control group was 26.09%. The differences between the two groups were significant （P 〈 0. 05）. Conclusion Treatment of perianal eczema with the combination of epinastine and 0. 1% tacrolimus ointment can achieve good curative effects with more safety and lower recurrence rate.
