海南发现2株新德里金属β-内酰胺酶-1阳性的阴沟肠杆菌 被引量:1

Identification of Two Strains of Enterobacter cloacae Carrying New Delhi Metallo-β-lactamase 1 Gene in Hainan

摘要 为了解临床分离的携带新德里金属β-内酰胺酶-1(NDM-1)基因的阴沟肠杆菌耐药性,笔者采用法国生物梅里埃公司的VITEK-2全自动细菌分析系统对临床分离的2株阴沟肠杆菌进行了鉴定,用IP/IPI E-test试纸条法协同试验检测了产金属酶类的碳青霉烯酶,用浓度梯度E-test条法进行药物敏感性试验,并对PCR产物进行电泳后纯化,测序比对确定基因型。结果表明,2株阴沟肠杆菌均携带β-内酰胺酶blaNDM-1基因,对美洛培南、亚胺培南、哌拉西林/他唑巴坦、头孢替坦、头孢吡肟、头孢曲松、头孢他啶、氨曲南、复方新诺明、阿米卡星等均有耐药性。由于分离出这2株携带NDM-1基因的阴沟肠杆菌的患者都是海南省居民,从未有国外旅行史,说明与国外报道的NDM-1肠杆菌科细菌没有流行病学关联,这对了解该基因在海南地区的流行情况具有重要意义。 Two strains of E. cloacae carrying New Delhi metallo-β-1actamase 1 (NDM-1) gene were isolated clin- ically and identified by using BioMerieux API identification system to study their drug susceptibility, and the NDM-1 gene from these two strains was identified by PCR. Metal producing enzyme carbapenemase was tested from these two strains by IP/IPI E-test and the drug susceptibility of the strains tested by concentration gradient E-test. The NDM-1 gene was detected in both of the two strains of E. cloacae, and the strains were found resist- ant to plenty of antibiotics, such as Meropenem, Imipenem, Pyocyanin, Cefotetan, Cefepime, Ceftriaxone, Ceftazidime, Aztreonam, Sulfamethoxazole and Amikacin. The NDM-1 gene plays an important role in drug sus- ceptibility. The patients infected with E. cloacae carrying the NDM-I are local people of Hainan Province, Chi- na, and they have not traveled abroad, suggesting no epidemiological link between this bacterium and that repor- ted overseas. The E. cloacaes identified in Hainan is endemic.
