开放式与闭合式牵引上颌阻生尖牙的CBCT研究 被引量:2

摘要 目的采用开放式与闭合式牵引矫治上颌阻生尖牙,应用锥形束CT研究两种方法矫治前后边缘骨以及根尖部牙槽骨的变化量,为临床选择治疗上颌阻生尖牙的开窗方法提供参考。方法选取29例上颌阻生尖牙患者,以采用开放式开窗的18例患者共18颗上颌阻生尖牙为开放组,另11例患者共14颗上颌阻生尖牙采用闭合式开窗为闭合组,所有患者均采用直丝弓矫治技术进行正畸牵引治疗,对牵引前后上颌阻生尖牙的边缘牙槽骨以及根尖部牙槽骨变化进行锥形束CT(CBCT)分析,采用SPSS 16.0软件对测量数据进行配对t检验。结果开放式牵引组唇、腭侧牙槽骨高度与矫治前比较,差异有显著性(P<0.05);闭合式牵引组唇、腭侧牙槽骨高度与矫治前比较,差异无显著性(P>0.05);开放式牵引组根尖部唇侧牙槽骨厚度与矫治前比较,差异有显著性(P<0.05);开放式牵引组根尖部腭侧牙槽骨厚度与矫治前比较,差异无显著性(P>0.05);闭合式牵引组根尖部唇、腭侧牙槽骨厚度与矫治前比较,差异无显著性(P>0.05)。结论上颌阻生尖牙采用闭合式牵引后边缘骨以及根尖部牙槽骨的吸收量小于开放式牵引组。 Objective Applying cone beam computed tomography(CBCT)to measure the marginal bone and the apex alveolar bone of the impacted maxillary canines treated with closed eruption versus open eruption.Methods Twenty nine patients with straight wire appliance orthodontic treatment of impacted maxillary canine were divided into 2 groups,one group chosen the treatment of closed eruption (CE) and the other group was open eruption (OE).The changes of the marginal bone and the apex alveolar bone of the impacted maxillary canine before and after treatment were measured with CBCT.Results There were significant differences on the alveolar bone height of the lip and palatal side for OE compared with the previous(P<0.05),but there were no significant differences on the alveolar bone height of the lip and palatal side for CE compared with the previous(P>0.05).There were significant differences on the thickness of the lip side of apex alveolar bone for OE compared with the previous (P<0.05),but there were no significant differences on the thick-ness of the palatal side of apex alveolar bone for OE compared with the previous (P>0.05),there were no significant differences on the thickness of the palatal and the lip side of apex alveolar bone for CE compared with the previous(P> 0.05).Conclusion The resorption of marginal bone and the apex alveolar bone of the CE group is less than OE group during clinical treatment of impacted canines.
