影像学检查和血清肿瘤标志物检测对肺癌诊断及病理分型的价值 被引量:4

摘要 目的探讨影像学检查和血清肿瘤标志物检测对肺癌诊断及病理分型的价值。方法收集2010年3月~2013年3月来我院就诊患者135例,其中经手术或穿刺病理确诊为肺癌患者83例,设为肺癌组,诊断为肺良性疾病组患者52例,设为良性组。选取同期正常体检者50例设为正常组,对所有研究对象进行影像学检查和血清肿瘤标志物检测。结果肺癌组NSE、SCC—Ag、Cyfra21—1单独检测与联合检测的阳性检出率均高于良性组与正常组(P〈0.01)。影像学检查,肺癌组的阳性率高于正常组(P〈0.01),但与良性组相比,较低,两组比较差异显著(X^2=23.59,P〈0.01),阳性检出率比较,肺癌组与良性组的联合检测(影像学检查+NSE+SCC—Ag+Cyfra21—1)均高于正常组(P〈0.05)。小细胞癌的NSE阳性率明显高于鳞癌、腺癌(P〈0.05)。鳞癌的SCC—Ag及Cyfra21—1阳性率明显高于腺癌、小细胞癌(P〈0.05)。而NSE+SCC—Ag+Cyfra21—1联合检查,影像学检查,影像学检查(+)NSE+SCC—Ag+Cyfra21三者间比较,差异无统计学意义(P〉0.05)。结论影像学检查和血清肿瘤标志物联合检查可以显著提高肺癌诊断的敏感性和准确性。 Objective To Explore the Imaging and serum tumor markers in diagnosis and pathological typing of lung cancer. Methods 135 patients in our hospital In 2010 March -2013 year in March were collected, which were diagnosed by operation or biopsy of 83 cases of lung cancer, for lung cancer group, 52 cases were diagnosed lung benign disease patients, as benign group. 50 cases the same normal physical examination were Select as normal group, all peoples were objectived with imaging and serum tumor markers. Results NSE in lung cancer group, SCC-Ag, Cyfra21-1 detection and combined detection of the positive detection rate was higher than the differences between benign group (P〈0.01). The positive rate of lung cancer imaging examination is higher than the normal group (P 〈 0.01), but lower than that in benign group(x2--23.59,P 〈 0.01),lung cancer group and benign group (combined imaging +NSE+SCC-Ag+Cyfra21-1) positive rate was higher than the normal group (P 〈 0.05). Squamous cell carcinoma,adenocarcinoma,small cell carcinoma were compared among the three groups,the positive rate of NSE in small cell carcinoma was significantly higher than that in squamous cell carcinoma, adenocarcinoma (P 〈 0.05). Squamous cell carcinoma SCC-Ag and Cyfra21-1 positive rate was significantly higher than that of adenocarcinoma, small cell carcinoma (P 〈 0.05). While the NSE+SCC-Ag+Cyfra21-1 joint examination, imaging examination, comparative examination + NSE+SCC-Ag+Cyfra21 seventy percent off sword image, no significant difference (P 〉 0.05). Conclusion Imaging and serum tumor markers joint inspection can significantly improve the sensitivity and accuracy in the diagnosis of lung cancer.
