调脂联合降压治疗对动态血压及血压变异性的影响

Influence of lipid joint antihypertensive treatment on ambulatory blood pressure and blood pressure variability

目的:探讨调脂药辛伐他汀联合降压药硝苯地平对高血压患者动态血压及血压变异性的影响。方法:选取226例诊断为高血压病的患者,按入院随机号分为两组,每组各113例,对照组单纯给予硝苯地平进行降压治疗;治疗组在此基础上联合辛伐他汀进行治疗;所有患者均于治疗前及治疗后行血脂及动态血压检查,记录24h平均收缩压及平均舒张压变化,并计算其均数标准差和24h动态脉压,比较两组治疗前后血脂及动态血压值及其稳定性的变化。结果:疗程结束后,观察组血脂水平明显低于对照组,差异具有统计学意义(P<0.01);观察组24h动态缩小脉压(24hPP)及收缩压变异率(SBPV)较对照组下降明显,两组比较,差异具有统计学意义(P<0.05);观察组舒张压变异率(DBPV)水平较治疗前明显下降,差异具有统计学意(P<0.05);且疗程结束后,观察组较对照组治疗后比较:收缩压标准差(24hSSD)、白昼收缩压标准差(dSSD)、白昼舒张压标准差(dDSD)、夜间收缩压标准差(nSSD)明显降低,差异有统计学意义(P<0.01)。结论:调脂药联合降压药可进一步提高血压的平稳性,值得临床广泛推广和应用。

Objective： To inve tensive drug nifedipine on ambulatory stiga bloo tension. Methods： A total of 226 patients into two groups. 113 cases in the contro d the impact of lipid-lowering drug simvastatin and antihyper- pressure and blood pressure variability in patients with hyper- iagnosed as hypertension were enrolled and randomly divided 1 group were treated with nifedipine antihypertensive therapy; treatment group were given nifedipine antihypertensive therapy and simvastatin. Lipids and ambulatory blood pressure were checked in all patients, 24h average systolic blood pressure and mean diastolic blood pressure changes were recorded, and mean standard deviation and 24h dynamic pulse pressure were calcu- lated. Ambulatory blood pressure values and their stability were compared before and after treatment. Re- suits. After treatment, the lipid levels of the observation group were significantly lower than the control group （P〈0.01） ; 24hPP and SBPV of the observation group decreased s control group （P〈05）. After the treatment, the 24hSD, dSSD, dDSD, significantly decreased （P 〈 0 01）. Conclusion. Lipid-lowering drug corn drugs can keep stable blood pressure for hypertension patients, and it＇s wort ignifican nSSD of binati hy of tly compared with the observation group on with antihypertensive application in large scale.
