超敏C反应蛋白与降钙素原在细菌感染中的相关性分析及诊断价值 被引量:22

摘要 目的:探讨超敏C反应蛋白(Hs—CRP)和降钙素原(PCT)在细菌感染中的相关性,并分析二者在细菌感染临床诊断中的应用价值。方法:随机选取2013年2~5月在我院诊治的165例细菌感染者及122例非细菌感染者作为研究对象,测定两组患者血中Hs—CRP与PCT的水平,分析两者相关性及其用于预测细菌感染的价值。结果:细菌感染组患者Hs—CRP、PCT水平及白细胞总数、中性粒细胞计数显著高于非细菌感染组(t分别为15.35、9.72、13.36、13.92,P均〈0.05)。细菌感染组患者HS—CRP与PCT之间存在相关性(r=0.826,P〈0.05)。预测细菌感染时,PCT的临界值为0.39rig/L,灵敏度、特异度分别为88.2%和77.1%;Hs—CRP的临界值为41mg/L,灵敏度、特异度分别为76.2%和71.5%;两者联合预测的灵敏度和特异度分别达到92.3%和81.5%。结论:Hs—CRP与PCT可作为细菌感染临床诊断的重要依据,两者联合检测可相互补充,具有重要的临床指导意义。 Objective. To analyze the correlation and diagnostic value of high sensitivity C reactive protein (Hs-CRP) and procalcitonin (PCT) in bacterial infection. Methods. A total of 165 cases with bac- terial infection and 122 cases with non bacterial infection from February 2013 to May 2013 were collected as observation group and control group, respectively, and the level of Hs-CRP and CRP were measured. The correlation of Hs-CRP and CRP and their diagnostic value were analyzed. Results- The level of Hs- CRP, PCT, WBC, neutrophil count in patients with bacterial infection were significantly higher than those of non bacterial infection group (t=15.35, 9.72, 13.36, 13.92, P〈0.05). There was positive correla- tion between Hs-CRP and PCT in the group of bacterial infection (r=0. 826, P〈0.05). The cutoff point, sensitivity and specificity to predict bacterial infection were as follow, PCT (0. 39 Mg /L, 88. 2% and 77.1%) ,CRP (41 mg/L, 76.2% and 71.5%). The sensitivity and specificity of prediction with PCT and CRP reached 92.3% and 81.5%, respectively. Conclusion. The detection of Hs-CRP and PCT can be used as an effective basis for clinical diagnosis of bacterial infection, and their joint detection can complement each other.
