妊娠高血压综合征剖宫产产妇凝血相关指标的变化

Changes of indicators of coagulation system in puerperae with pregnancy-induced hypertension undergoing cesarean section surgery

目的:观察妊娠高血压综合征对剖宫产产妇凝血相关指标变化的影响。方法:选取来我院进行剖宫产的妊娠高血压综合征产妇68例作为观察组,选取同时期的正常产妇68例作为对照组。于剖宫产前及产后1周,对两组产妇的凝血系统功能指标包括凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、凝血酶时间(TT)、纤维蛋白原(Fib)及血清D-二聚体(D-D)、胱抑素C、肌酐水平进行测定,比较组间差异。结果:观察组剖宫产前及产后1周PT、APTT、TT均短于对照组,血浆中Fib含量显著高于对照组,差异均具有统计学意义(P<0.05);观察组产后1周与剖宫产前相比,PT、APTT、TT升高,而血浆中Fib含量则下降,差异具有统计学意义(P<0.05);观察组剖宫产前及产后1周血清D-二聚体、胱抑素C、肌酐水平均高于对照组,差异具有统计学意义(P<0.05);观察组产妇产后1周与剖宫产前相比,血清D-二聚体、胱抑素C、肌酐水平下降,差异具有统计学意义(P<0.05)。结论:妊娠高血压综合征剖宫产产妇凝血相关指标明显异于正常剖宫产产妇,而且产前、产后波动较大。

Objective= To observe changes of indicators of coagulation system in puerperae with pregnancy-induced hypertension undergoing cesarean section surgery. Methods= A total of 68 puerperae with pregnancy-induced hypertension undergoing cesarean section surgery in our hospital were selected as observation group, and 68 healthy puerperae were selected as control group. One week before and after undergoing the surgery, indicators of coagulation system function such as PT, APTT, TT, Fib, serum level of D-D, cystatin C, and creatinine were detected and compared between the two groups. Results. One week before and after undergoing cesarean section surgery, the PT, APTT, TT of observation group were significant shorter, but Fib concentration was higher than those of the control group （P〈0.05）. In the observation group, one week after undergoing the surgery, PT, APTT, and TT increased but serum Fib, level of D-D, cystatin C, and creatinine levels decreased significantly, comparing with that before the sur- gery （P〈0.05）, however, the serum level of D-D, cystatin C, and creatinine levels were significant high- er than that of the control group （P〈0. 05）. Oonclusion：For puerperae with pregnancy-induced hyperten- sion shows abnormal changes in indicators of coagulation system comparing with healthy puerpere, and these indicators obviously fluctuate before and after undergoing cesarean section.
