髋关节肿瘤假体置换治疗股骨近端转移瘤疗效观察

Observation on efficacy of prosthetic replacement for metastatic tumor of proximal femur

目的 探讨髋关节肿瘤假体置换治疗股骨近端转移瘤的临床疗效.方法 采用髋关节肿瘤假体治疗24例股骨近端转移瘤患者,并对其治疗后肢体功能、生活质量进行评价分析.结果 24例患者安全度过围手术期；Karnofsky评分：术前平均为（36.25±17.98）分,术后6个月为（51.25 ±16.91）分,差异有统计学意义（t=8.8,P＜0.05）,术后患者生活质量明显改善.按照En-neking评分系统评估术后患肢功能,平均（24.04 ±3.02）分,优良率79.17％.随访期间无假体周围感染、松动及断裂事件发生.结论 肿瘤假体置换治疗股骨近端转移瘤可减轻患者疼痛、提高生活质量.

Objective To evaluate the clinical effect of prosthetic replacement in the treatment of metastatic tumor of proximal femur.Methods The artificial prosthesis replacement was carried out in 24 patients with metastatic tumor of proximal femur.After the surgery,the physical function and quality of life score were analysed.Results All patients were in good condition during the perioperative period.The preoperative and postoperative Karnofsky score were（36.25 ± 17.98）and （51.25 ± 16.91）respectively （t =8.8,P ＜ 0.05）,and the postoperative quality of patient＇s life was significantly improved.According to the Enneking system,the postoperative mean score was （24.04 ± 3.02）,and the overall excellent and good function rate was obtained in 79.17％ of the patients.No endoprosthetic infection,loosening or fracture happened.Conclusion Artificial prosthesis replacement in the treatment of metastatic tumor of proximal femur can relieve the pain and increase the life quality of patients.
