益气养阴消癥通络中药对早期糖尿病肾病大鼠24h尿蛋白及nephrin、podocin基因表达的相关性分析 被引量:8

摘要 目的观察早期糖尿病肾病大鼠24 h尿蛋白(U Pro)及肾组织中nephrin、podocin基因表达,探讨nephrin、podocin分子在糖尿病肾病蛋白尿发生过程中的相互作用。方法将SD雄性大鼠随机分为6组:肾切组、模型组、厄贝沙坦组、中药低、中、高剂量组,各组相应药物剂量灌胃。于第6周末检测各组大鼠24 h U Pro量、肌酐(Scr)和尿素氮(BUN)水平;反转录聚合酶链反应(RT-PCR)测肾皮质nephrin、podocin表达水平。结果厄贝沙坦组、中药低、中剂量组与模型组比较,U pro量显著下降(P<0.05)。各治疗组与模型组比较,Scr、BUN明显降低(P<0.05),且nephrin、podocin表达显著增加(P<0.01);24 h U Pro与nephrin、podocin表达量呈负相关,nephrin与podocin表达量呈正相关。结论糖尿病大鼠肾小球上皮细胞足突nephrin、podocin表达的减少可能是糖尿病肾病蛋白尿发生的机制之一。 Objective To observe gene expressions of nephrin and podocin in renal tissue in rats with diabetic nephropathy,and to discuss the interaction of nephrin and podocin on proteinuria of diabetic nephropathy.Methods Male Sprague-Dawley (SD) rats were randomly divided into nephrectomy group,model group,irbesartan group,high-dose,mid-dose and low dose Chinese medicinal groups (high-dose group,mid-dose group and low-dose group).All groups were given relevant medicinal intragastrically.At the end of the 6th week,the levels of 24-hour urinary protein (U Pro) Scr and BUN were detected in rats of all groups,and expressions of nephrin and podocin in renal cortex were detected by using reverse transcription polymerase chain reaction (RT-PCR).Results The level of U Pro decreased significantly in irbesatan group,low-dose group and mid-dose group compared with that in model group (P < 0.05).The levels of Scr and BUN were improved and the expressions of nephrin and podocin increased significantly in all treatment groups compared with those in model group (P < 0.05).The results demonstrated a negative correlation between 24-hour U Pro and expressions of nephrin and podocin,and a positive correlation between expression of nephrin and that of podocin.Corclusion The decrease of expressions of nephrin and podocin in glomerular epithelial cells may be one of proteinuria mechanisms of diabetic nephropathy in rats with diabetes.
