针灸治疗女性膀胱过度活动症的临床研究

Treatment of female overactive bladder with acupuncture

目的探讨针灸治疗女性膀胱过度活动症(OAB)的临床疗效。方法 60例女性OAB患者随机分为针灸组和索利那新组。以中文版OAB症状评分问卷调查表(OABSS)对OAB的症状进行量化,以评价OAB的严重程度;以排尿日记记录治疗前后24 h平均尿急次数、日间排尿次数、夜尿次数、每次尿量、尿失禁次数中位数,以观察治疗效应。索利那新组30例,每日口服索利那新5 mg1次,共4周;针灸组30例,针刺关元、肾俞、足五里、阴陵泉、水道、中极、中膂俞、膀胱俞、次髎,每日1次,共4周。运用SPSS 13.0软件处理数据,对其疗效进行分析。结果治疗4周后针灸组总有效率为90%,索利那新组总有效率86.9%,2组疗效相当(P>0.05)。各组治疗后24 h平均尿急次数、日间排尿次数、夜尿次数、每次尿量、尿失禁次数中位数均比治疗前有明显改善(P<0.05),且针灸组对尿急的治疗效果优于索利那新组(P<0.05),而2组日间排尿次数、夜尿次数、每次尿量、尿失禁次数中位数疗效比较无统计学意义(P>0.05)。结论针灸治疗能安全有效地改善女性OAB的症状,可作为一种有效的治疗手段。

Objective To investigate the curative effect of acupuncture on female overactive bladder （OAB）.Methods The female patients （n =60） with OAB were randomly divide into acupuncture group and solifenacin group.The symptoms of OAB were quantified with the questionnaire （Chinese version） of OAB symptom scores （OABSS） for reviewing the severity of OAB.The average frequency of urgency to urination in 24 hours,daytime urination frequency,nighttime urination frequency,urine volume of every time and median of uroclepsia frequency were recorded with urination diary for observing curative effect.The solifenacin group （n =30） was orally given solifenacin （5 mg） per day for 4 weeks,and acupuncture group （n =30） was needled in Guanyuan （CV4）,Shenshu （BL23）,Zuwuli （LR10）,Yinlingquan （SP9）,Shuidao （ST28）,Zhongji （CV3）,Zhonglvshu （BL29）,Pangguangshu （BL28） and Ciliao （BL32） once a day for 4 weeks.All data was treated by using SPSS 13.0 software and the curative effect was analyzed.Results The total effective rate was 90％ in acupuncture group and 86.9％ in solifenacin group （P ＞ 0.05）.After treatment the average frequency of urgency to urination in 24 hours,daytime urination frequency,nighttime urination frequency,urine volume of every time and median of uroclepsia frequency were all decreased in two groups than before （P ＜ 0.05）.The curative effect on urgency to urination was better in acupuncture group than that in solifenacin group （P ＜ 0.05）,and had no statistical significance in the comparison of daytime urination frequency,nighttime urination frequency,urine volume of every time and median of uroclepsia frequency between two groups （P ＞ 0.05）.The difference in OABSS had significance in two groups before and after treatment （P ＜ 0.05） and had no significance after treatment （P ＞ 0.05）.Conclusion Acupuncture can relieve safely and effectively the symptoms of OAB in female patients.
