雾化吸入布地奈德在AECOPD中的疗效分析 被引量:15

摘要 目的探讨布地奈德雾化吸入治疗老年慢性阻塞性肺病急性加重期(AECOPD)的临床应用价值。方法 64例老年AECOPD患者随机分为2组,布地奈德组(34例)和甲强龙组(30例),两组均在常规治疗基础上分别使用布地奈德雾化吸入和甲泼尼龙静脉治疗。对两组患者的治疗效果及不良反应进行比较分析。结果①两组患者治疗前后PaO2、PaCO2、SaO2、FEV1、CAT评分均有明显改善,差异具有统计学意义(P<0.05),但两组间比较无差异(P>0.05)。②不良反应方面,布地奈德组主要发生声嘶,甲强龙组主要发生血糖升高。结论布地奈德雾化吸入在治疗老年AECOPD中疗效可靠,减少了糖皮质激素全身使用的副作用,值得临床推广使用。 Objective To investigate the clinical efficacy of budesonide atomizing inhalation in elderly patients with AECOPD. Methods Collected 64 cases of AECOPD were randomly divided into two groups. On the basis of the treatment, one group (34 cases) were treated with budesonide, another group (30 cases) were treated with methylprednisolone. The clinic score, pulmonary function ( FEVj ) , blood gas analysis( PaO2 , PaCO2, SaO2 ) and adverse effects were recorded, compared then analyzed. Results The clinical effect after treatment of budesonide group and methylprednisolone group were better than that of before, and the lung function and blood gas analysis results were better too. However, there was no significant difference between the two groups. Conclusions AECOPD treat- ment using budesonide atomizing inhalation could improve lung function and had reliable effect. It could reduce the adverse effect of taking glucocorticoid orally, so it was worthy of promoting in the clinic.
