高危妊娠孕妇剖宫产术前心理状况及相关社会心理因素分析

Analysis of cesarean section in psychological status before operation and social psychological factors of high- risk pregnancy

目的：探讨与分析高危妊娠孕妇剖宫产术前心理状况,并了解相关社会心理因素.方法：选取2012年1月～2013年1月我院妇产科收治的150例高危妊娠孕妇为研究对象,根据其是否存在心理问题将其分为对照组87例与心理组63例.将两组患者的临床资料进行收集、统计,采用单因素及多因素分析影响高危妊娠孕妇剖宫产术前的心理因素.结果：本组150例高危妊娠孕妇中,SDS评分为（61.72±18.26）分,SAS评分为（59.61±16.49）分.高危妊娠孕妇剖宫产术前心理问题的发生与孕前思想准备、分娩恐惧、担心分娩安全、担心原有不良妊娠史、担心某些家族疾病史会影响到胎儿、分娩知识了解及待产准备等因素有关.Logistic多因素逐步回归分析显示,担心分娩安全是高危妊娠孕妇的独立危险因素,待产准备则为保护因素.结论：为了降低不良因素对高危妊娠孕妇的影响,高危妊娠孕妇在剖宫产术前需做好待产准备,并做好相关的健康指导,让孕妇对分娩有个正确的认识,降低孕妇对胎儿安全的担心.

Objective：To study and analyze the risk cesarean section pregnancy psychological status before operation, and understanding of the relevant social psychological factors. Methods： From January 2012 to January 2013 in our hospital treated 150 cases of high - risk pregnant women as the research objects, according to the presence of psychological problems may be divided into control group with 87 ca- ses and psychological group with 63 cases. The clinical data of two patients were collected, statistics, single factor and multi factor analy- sis in high risk pregnancy cesarean section preoperative psychological factors. Results： In this group, 150 cases of high -risk pregnant women, SDS score was （61.72 ± 18.26）I,,SAS was （59.61 ± 16.49）. High risk pregnancies before cesarean section of psychological problems and mental preparation, before delivery fear, worry about the safety of childbirth pregnancy history, fear that some family history of disease will affect the fetus, delivery knowledge and labor preparation factors. Multiple stepwise regression analysis showed Logistic, worried about the safety of childbirth was an independent risk factor for pregnant ~romen of high risk pregnancy, labor ready for protective factors. Conclusion ： In order to reduce the impact of adverse factors of high - risk pregnancy, high - risk pregnant women during cesare- an section should be prepared before labor, and do a good job of health guidance related to pregnant women, to have a correct under- standing of delivery, reduce matemal to fetal safety concerns.
