早期集束化治疗感染性休克的临床分析

The Clinical Curative Effect of Early Intensive Medicine Bundle Therapy in Septic Shock

目的探讨重症医学科早期集束化治疗感染性休克的临床疗效。方法将2011年1月至2012年10月由泸州市中医医院重症医学科收治的60例感染性休克患者随机分为集束化治疗组和传统治疗组,各30例,集束化治疗组,使患者6 h达到早期复苏目标,传统治疗组采取传统经验的治疗方法。分析6、24、48 h感染性休克集束化治疗各指标[平均动脉压(MAP)、中心静脉压(CVP)、中心静脉血氧饱和度(ScvO2)]与预后的关系,分别记录两组的多器官功能障碍(MODS)发生率、病死率、心率、平均住院时间。结果集束化治疗组患者的MODS发生率、病死率、心率、平均住院时间显著低于对照组(P<0.05),集束化治疗组MAP、CVP、ScvO2显著高于传统治疗组(P<0.05)。结论早期集束化治疗感染性休克可有效提高感染性休克的抢救成功率,疗效确切。

Objective To investigate the clinical curative effect of early intensive medicine bundle ther- apy in septic shock. Methods From Jan. 2011 to Oct. 2012 60 patients with septic shock from Emergency Department of Luzhou People＇s Hospital were randomly divided into bundle therapy treatment group and con- ventional treatment group,30 cases in each group. The bundle therapy treatment group completely treated with bundle therapy, to achieve 6 hours early recovery goal ,30 cases in the conventional treatment group were given conventional treatment, of the correlation between various indicators such as mean arterial pressure （MAP） , centrM venous pressure （CVP） , central venous oxygen saturation （ ScvO2 ） and prognosis at 6 h,24 h and 48 h of septic shock cluster treatment were analyzed;MODS incidence,mortality rate, heart rate,average hospitalization time of the two groups were recorded. Results MODS incidence,case fatality rate,the mean duration of hospitalization of the bundle therapy treatment group were significantly lower than that of the con- ventional treatment group（ P 〈 0.05 ） ; CVP MAP, ScvO2 of the bundle therapy treatment group were signifi- cantly higher than that of the conventional treatment group（ P 〈 O. 05）. Conclusion Early bundle therapy ean effectively improve the success rate of septic shock rescue with affirmative effect.
