ARV药物毒副反应老年患者生活质量及其影响因素回顾性研究

Retrospective research on the quality of life and the affected factors among the older AIDS patients with side effects by ARV medicine

目的:探讨服用抗反转录病毒(ARV)药物后出现副反应的老年艾滋病患者生活质量及其影响因素,为临床诊治提供依据。方法:采用回顾性研究,分析总结年龄≥50岁老年组患者生活质量及影响因素。结果:共纳入分析病例92例。主要的不良反应病症有血液毒性反应、消化道毒副反应及高脂血症。患者的总的生活质量和健康状况明显低于普通人群,生理、心理及社会关系方面受到艾滋病的影响更为明显(P<0.05)。女性在独立性领域及社会关系领域得分低于男性,其他领域得分高于男性,差异不显著。回归分析结果提示年龄、性别、婚况、吸烟、饮酒、文化水平、CD4+T分段、不同病症、治疗方案对生活质量得分影响较小(F=1.15,P>0.05)。结论:年龄≥50岁的艾滋病合并药物毒副反应患者的总的生活质量和健康状况明显低于普通人群,未发现年龄、性别、文化水平、CD4+T分段、不同病症、治疗方案等对患者生活质量有显著影响。

Objective： Research on the quality of life （QOL） and the affected factors among the older AIDS patients with adverse reactions by ARV medicine in order to give reference for the clinical therapy. Methods： Analysis on the QOL and affected factors among older AIDS patients by retrospective study. Results： 92 cases were collected in the study, and the adverse reactions included hematotoxicity, gastrointestinal reactions and hyperlipidemia. The QOL and health status of patients were lower than normal people. And the patients with AIDS were affected more obviously in the physical, psychological and social relationship domain （P〈0.05）. The score of female in independent and social domain was lower than male, and in the other domains was higher, but there was no significance difference. The result of regression analysis showed that there were a little effects on the scores of the QOL in age, gender, marriage state, smoking, alcohol drinking, education level, CD4 cell counts, different diseases and symptoms, and the treatment schedule （F=l.15, P〉0.05）. Conclusion： The QOL and health status of patients are lower than normal people. There are a little effects on the scores of the QOL in age, gender, marriage state, smoking, alcohol drinking, education level, CD4 T cell counts, different diseases and symptoms, and the treatment schedule.
