妊娠期高血压疾病预测分析系统的临床应用

摘要 目的:探讨妊娠期高血压疾病预测分析系统在妊娠期高血压疾病中的临床应用情况,观察妊娠期高血压疾病预测阳性患者接受干预性治疗的临床效果。方法应用SD-Ⅱ型妊娠期高血压疾病预测分析系统对江阴市人民医院产科门诊1364例孕20~40周孕妇进行监测,预测阳性者每2周复查1次,预测阴性者每4周复查1次,对预测阳性患者部分进行生活指导和干预性治疗,为干预组,非干预组在出现高血压时再干预,观察两组发生妊娠期高血压的情况。结果有32例患者妊娠期高血压疾病预测阳性,预测阳性组的血管阻力(VAR)(1.59±0.09)、血液黏度(BV)(4.83±0.12)、心排指数(CI)(4.15±0.05)比预测阴性组(VAR=1.17±0.06,BV=4.07±0.03,CI=3.23±0.04)高,差异有统计学意义(F值分别为450.274、1220.253、6013.051,均P=0.00)。干预组的妊娠期高血压疾病发生率为0(0/16),而非干预组妊娠期高血压疾病发生率为18.75%(3/16),非干预组妊娠期高血压疾病发生率明显高于干预组,但差异无统计学意义( P>0.05)。结论妊娠期高血压疾病预测分析系统通过对妊娠期高血压疾病的预测,及时对患者进行生活指导和干预性治疗,对于减少妊娠期高血压疾病的发生发挥了很大的作用。 Objective To discuss the clinical application of hypertensive disorders in pregnancy ( HDP) forecast and analysis system and observe the interventional treatment effect on HDP patients with positive predictive value .Methods SD-ⅡHDP forecast analysis system was adopted to monitor 1 364 pregnant women with 20-40 weeks of gestation admitted in obstetrics department of Jiangyin People Hospital .For HDP positive patients recheck was done every 2 weeks, while for HDP negative patients every 4 weeks.Life style guidance and interventional measures were provided for some of forecast HDP positive patients ( intervention group ) , while interventional measures were taken for non-intervention group when hypertension occurred .Then the HDP status of two groups was observed .Results There were 32 positive patients, and the values of vascular resistance (VAR)(1.59 ±0.09), blood viscosity(BV)(4.83 ±0.12), cardiac index(CI) (4.15 ±0.05) of forecast positive group were higher than those of forecast negative group (VAR=1.17 ±0.06, BV=4.07 ±0.03, CI=3.23 ±0.04).The differences were statistically significant ( F value was 450.274, 1 220.253 and 6 013.051, respectively, all P=0.00).The incidence rate of HDP in intervention group was 0(0/16), while that was 18.75% (3/16)in non-intervention group.The incidence rate of non-intervention group was obviously higher than intervention group , but the difference was significant ( P〉0.05 ) . Conclusion With forecast by HDP forecast and analysis system , life style guidance and interventional measures can be given in time , which plays a great role in reducing the incidence of HDP .
