独立分娩室联合家属及导乐陪伴分娩的临床分析

目的探讨独立分娩室联合家属、导乐陪伴分娩模式对初产妇分娩结果的影响。方法选择在瑞安市人民医院独立分娩室联合家属、导乐陪伴分娩的初产妇300例(观察组)和普通分娩的初产妇290例(对照组),比较两组产妇的分娩方式、各产程时间、产时及产后2h出血量、新生儿Apgar评分及产妇疼痛情况。结果观察组产妇自然分娩率为86.6%,与对照组相比分娩方式差异有统计学意义(x^2=15.37,P<0.01);观察组产妇的第一、二、三产程较对照组缩短,产时及产后2h出血量较对照组减少,差异均有统计学意义(t值分别为8.612、8.426、3.546、5.867,均P<0.01);观察组新生儿Apgar评分高于对照组(t=3.656,P<0.01);观察组产妇产时的疼痛得分较对照组低,差异有统计学意义(t=3.964,P<0.01)。结论独立分娩室联合家属、导乐陪伴分娩模式对分娩结局可产生积极作用,可加快产程,减少产后出血量,增加新生儿Apgar评分,且能减轻产妇分娩时的疼痛,是目前临床上安全的、值得推广的分娩模式。

Objective To explore the effects of Doula delivery in independent delivery room and accompanied by relative on delivery results of primiparas.Methods Three hundred primiparas giving birth in independent delivery room with relative accompanying and Doula delivery were selected in observation group in the People ＇ s Hospital of Ruian in Zhejiang Province .And 290 cases of primiparas with ordinary delivery were included in control group .Two groups were compared in delivery mode , time of each labor stage , blood loss during labor and 2h after delivery, neonatal Apgar score and maternal pain .Results The natural delivery rate was 86.6% in the observation group, and there was significant difference between two groups （χ2 =15.37, P〈0.01）.Compared with the control group , the first, second and third labor of the observation group was shorter , and the blood loss at labor and 2h after delivery was less .The differences were statistically significant（t value was 8.612, 8.426, 3.546 and 5.867, respectively, all P〈0.01）.The neonatal Apgar score in the observation group was higher than the control group （t=3.656,P〈0.01）.The score of maternal pain of the observation group was lower than the control group, and the difference was statistically significant （t=3.964,P〈0.01）.Conclusion Doula delivery in independent delivery room and accompanied by relative has positive effects on shortening the stages of labor , reducing postpartum hemorrhage , increasing neonatal Apgar score and reducing the pain of the primiparas .It is a safe mode of delivery deserving popularizing .
