低钾性周期性麻痹患者红细胞内外钾、血液pH和Na^+-K^+-ATP酶活性的相关性研究 被引量:1

摘要 目的通过检测低钾性周期性麻痹患者治疗过程中的红细胞内外钾浓度、血液pH值和Na+-K+-ATP酶活性,观察其血液pH值的变化对红细胞内外钾浓度的影响,及Na+-K+-ATP酶活性的变化对红细胞内外钾浓度变化的影响,并进一步探讨其各项指标的相关性。方法采用磷钼酸比色法测定46例低钾性周期性麻痹患者发作期和恢复期及40例健康人的Na+-K+-ATPase(简称NKA),同时测定患者发作期和恢复期血pH值、红细胞内钾和红细胞血清钾等项目。结果①周期性麻痹患者发作期NKA显著高于恢复期组和正常对照组(P<0.01);恢复期组和对照组差异无统计学意义(P>0.05);②发作期pH值显著高于恢复期(P<0.01);③发作期红细胞内钾和恢复期比较,差异无统计学意义(P>0.05);发作期血清钾和恢复期血清钾比较,差异有统计学意义(P<0.01)。结论发作期NKA活性增加可导致血K+降低,同时引起血pH值降低;红细胞内钾的浓度变化不大,但会影响pH值的改变。 Objective To observe the influence of changes of blood pH and enzymatic activity of Na^-K^-ATP on potassium concentration in plasma and erythrocytes by testing potassium level in plasma and erythrocytes,blood pH,and enzymatic activity of Na+-K+-ATP in patients with hypokalaemic periodic paralysis in order to further discuss the cor- relation among each index. Methods The Na-K-ATPase (NKA),blood pH,and potassium in plasma and erythrocytes at stages of attack and recovery in 46 patients with hypokalaemic periodic paralysis were tested as well as NKA in 40 healthy participants by phosphomolybdic acid colorimetry. Results The value of NKA at stage of attack in patients with hypokalaemic periodic paralysis was obviously higher than that of recovery group and control group (P〈0.01),but there was no significant difference in comparison of recovery group and control group (P〉0.05).The pH value at stage of attack was remarkably higher than that of the recovery group (P〈0.01).At stage of attack,potassium level in eythro- cytes was not different from that of the recovery group (P〉0.05),whereas there was a statistical difrencc in potassium level in plasma at stage of attack and recovery (P〈0.01). Conclusion Enhancement of NKA activity at stage of attack leads to reductions of blood potassium and blood pH.The potassium concentration in erythrocytes changes little,but it will affect the changes of blood pH.
