胃部分切除术在早期胃癌治疗中的临床价值

Clinical value of partial gastrectomy in the treatment of early gastric cancer

目的 分析胃部分切除术在早期胃癌治疗中的临床疗效.方法 选取本院2006年1月～2008年5月收治的早期胃癌患者52例,随机分为对照组与观察组各26例,对照组患者接受常规根治术治疗,观察组患者进行胃部分切除术治疗,对比两组的临床疗效.结果 两组患者治疗后,观察组胃肠通气时间、胃排空时间少于对照组,6个月体重增加值高于对照组,差异有统计学意义（P＜0.05）;观察组患者并发症为胆汁反流性胃炎,对照组并发症主要为胆汁反流性胃炎与倾倒综合征,观察组术后并发症发生率显著低于对照组（P＜0.05）.两组患者术后复发率、5年生存率差异无统计学意义（P＞o.05）.结论 在早期胃癌治疗中,胃部分切除术可显著缓解患者症状,有效延长患者生存期,大幅提高患者生存质量,临床疗效显著,值得在临床中推广.

Objective To analyze the clinical curative effect of partial gastrectomy in the treatment of early gastric cancer.Methods 52 cases of patients with early gastric cancer, who were treated in our hospital from January 2006 to May 2008 were randomly divided into control group and observation group with 26 cases in each group.Patients in control group were given conventional radical operation,and patients in observation group were given partial gastrecto- my.The clinical curative effect was compared between two groups.Results After treatment,the gastrointestinal ventila- tion time,gastric emptying time of the observation group was lower than that of the control group, weight gain in 6 months was significantly higher than that of control group respectively,the difference was significant （P〈0.05）.The main complications in observation group was bile reflux gastritis,and that in control group was bile retlux gastritis and dumping syndrome,the complication rate in observation group was lower than that of the control group （P〈0.05）.There was no significant ditterence between patients in two groups on postoperative recurrence rate and 5-year survival rate （P〉0.05）.Conclusion In the treatment of early gastric cancer, partial gastrectomy can significantly alleviate the symp- toms of patients,eftectivcly prolong survival,and significantly improve the quality of life.The clinical curative effect is remarkable,so it is worth popularization in clinic.
